 Description
 
 
 
 Gateway Rehab Center (GRC) has an outstanding opportunity for a Nurse Liaison Gateway Rehab who will be responsible for the pre-admission case management, ASAM level of care assessment, and coordination of admission to care for substance use disordered patients referred from a hospital setting.
 
 To be considered for the position, you must live within the Pittsburgh, PA area or surrounding counties.
 
 
 
 
 Responsibilities
 
 
 
 <ul>
 <li>
 Assesses admission candidates&rsquo; medical and psychiatric appropriateness for treatment.
 </li>
 <li>
 Determines level of care placement based on ASAM criteria.
 </li>
 <li>
 Pre-certifies admissions as required.
 </li>
 <li>
 Discusses treatment options with referral sources.
 </li>
 <li>
 Acts as liaison between Gateway and outside referral sources.
 </li>
 <li>
 Coordinates patient transfers from other facilities to Gateway Aliquippa/Westmoreland.
 </li>
 <li>
 Responds to needs of referral sources and managed care representatives.
 </li>
 <li>
 Interacts with the physician through coordination of patient assessments. Attends GRC mandatory training and in-services.
 </li>
 <li>
 Other duties as required.

 </ul>
 
 Knowledge, Skills, And Abilities
 
 
 
 <ul>
 <li>
 Strong communication skills required.
 </li>
 <li>
 Able to work independently with minimal oversight.
 </li>
 <li>
 Knowledge of skilled nursing
 
 
 </li>
 </ul>
 
 Requirements
 
 
 
 <ul>
 <li>
 Pennsylvania RN or LPN licensure
 </li>
 <li>
 3+ years nursing experience preferred.
 </li>
 <li>
 Experience identifying/treating drug and alcohol addictions.
 </li>
 <li>
 Experience in conducting assessments and evaluations.
 
 
 </li>
 </ul>

 <li>
 Pass PA Criminal Background Check
 </li>
 <li>
 Obtain PA Child Abuse and FBI Fingerprinting Clearances.
 </li>
 <li>
 Pass Drug Screen
 </li>
 <li>
 TB Test
 </li>
 <li>
 Access to reliable and dependable internet connection.
 
 
 </li>
 </ul>
