Summary of the Primary Care Physician role:</b><b> </b>The older adults we serve experience challenges with social determinants of health and living, which impact their health. The Primary Care Provider will guide our patients to build their best lives according to their goals. They do this by providing a full spectrum of care while utilizing state-of-the-art technology and establishing trusted longitudinal care in our clinics and via telehealth. Working with other members of the care team, our &quot;person-first&quot; approach strives to create both individual and community wellness.</p><h3 class="jobSectionHeader"><b>
Day-to-day duties:</b></h3><ul><li>
Participate in our daily team huddles to discuss patient concerns and care plans</li><li>
Perform in-person exams as well as telehealth visits, supported by our EMTs who are in our patients&rsquo; homes</li><li>
Collaborate closely with our &ldquo;Gather Guide Partners&rdquo; who are part of the care team and work to address the patients&rsquo; social needs</li><li>
Communicate and follow up with emergency room providers regarding our patients&rsquo; history, needs, and care</li><li>
Communicate with patients to remain close to their health journey and provide support</li><li>
Document visits and clinical notes in our EHR (Athena)</li><li>
Provide on-call support for some escalated cases</li></ul>
Requirements:
<h3 class="jobSectionHeader"><b>Minimum Requirements:</b></h3><ul><li>
BC/BE in Internal Medicine or Family Medicine (must obtain board certification within one-year from start date)</li><li>
An active medical license in the Commonwealth of Massachusetts, or the ability to obtain one prior to start date</li><li>
Clinical proficiency in evidence-based primary care</li><li>
Ability to communicate and connect with patients from a diverse set of backgrounds</li><li>
Proven history of working successfully with EHR and other relevant technologies/tools</li><li>
Strong ethics with the ability to navigate through complex and highly sensitive situations with confidentiality</li><li>
Desire to be a part of a mission-driven organization with a focus on improving lives and communities</li><li>
Ability to work well with others in a non-hierarchical environment where opinions are welcomed regardless of titles</li><li>
Ability to remain calm when balancing conflicting priorities</li></ul><h3 class="jobSectionHeader"><b>
