Summary of the Primary Care Partner role: The older adults we serve experience challenges with social determinants of health and living, which impact their health. The Primary Care Partner will guide our patients to build their best lives according to their goals. They do this by providing a full spectrum of care while utilizing state-of-the-art technology and establishing trusted longitudinal care in our clinics and via telehealth. Working with other members of the care team, our &quot;person-first&quot; approach strives to create both individual and community wellness.

Minimum Requirements:<ul><li>
Board Certification</li><li>
An active medical license in the Commonwealth of Massachusetts, or the ability to obtain one prior to start date</li><li>
Clinical proficiency in evidence-based primary care</li><li>
Ability to communicate and connect with patients from a diverse set of backgrounds</li><li>
Proven history of working successfully with EHR and other relevant technologies/tools</li><li>
Strong ethics with the ability to navigate through complex and highly sensitive situations with confidentiality</li><li>
Desire to be a part of a mission-driven organization with a focus on improving lives and communities</li><li>
Ability to work well with others in a non-hierarchical environment where opinions are welcomed regardless of titles</li><li>
Ability to remain calm when balancing conflicting priorities</li></ul>
Preferred Experience:<ul><li>
