 We are looking for a compassionate and skilled
 
 Registered Nurse (RN)
 
 in the
 
 Howardwick, TX
 
 area to administer patient care in accordance with a physician-established care plan. This role is a 1099 contract opportunity, with care being provided at the patient&rsquo;s residence.
 
 Patient's needs include 4 hours/month, prefers visits on Fridays
 
 .
 
 
 
 Responsibilities
 
 
 
 <ul>
 <li>
 Execute physician-prescribed plans of care
 </li>
 <li>
 Complete compliant documentation of the care provided in the system of record
 </li>
 <li>
 Manage and administer prescribed medication, treatment, and therapies
 </li>
 <li>
 Perform various activities associated with daily living
 </li>
 <li>
 Monitor for changes in the patient's condition, such as weight loss/gain, self-care abilities, and indicators of disease progression
 </li>
 <li>
 Communicate with family members, physicians, case managers, and other prescribed individuals regarding the patient's health
 </li>
 <li>
 Work with patients is done one-on-one in the patient's home to provide a comfortable environment
 </li>
 <li>
 Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants
 </li>
 <li>
 Perform other personal care services as necessary to meet the patient's needs

 </ul>
 
 Minimum Qualifications
 
 
 
 <ul>
 <li>
 Current State or Compact License as a Registered Nurse
 </li>
 <li>
 1+ years of experience as an RN in a clinical or home health setting
 </li>
 <li>
 Current CPR certification
 </li>
 <li>
 Willing to get TB tested
 </li>
 <li>
 Maintain required insurance
 </li>
 <li>
 Must have dependable transportation and be willing to commute to the patient&rsquo;s home

 We are looking for a compassionate Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in Espanola, NM to provide assistance in daily living and personal care services in patients&rsquo; homes in accordance with a physician established care plan.
 
 
 This role is a 1099 contract opportunity, with care being provided at the patient&rsquo;s residence. The current scheduling needs for this position will be established between the HHA/CNA, patient, and case manager.
 
 
 Available Shifts:
 
 
 8hrs a day (48hrs a week) Tuesday, Wednesday, Thursday, Friday, Saturday, Sunday
 
 
 Mid-Late Morning
 
 
 Offers are contingent upon passing a background check.
 
 
 
 Responsibilities:
 
 
 
 <ul>

 Work with the patient in their home to provide one-on-one service in a comfortable and familiar setting
 </li>
 <li>
 Assist with the patients&rsquo; activities of daily living as well as personal care
 </li>
 <li>
 Light housekeeping
 </li>
 <li>
 Meal preparation
 </li>
 <li>
 Monitor vitals (vital signs, temperature, respiration, etc) according to plan of care
 </li>
 <li>
 Remind patient of to take medication as prescribed
 </li>
 <li>
 Maintain records and documentation of services
 </li>
 <li>
 Communication with the patients&rsquo; family, doctors, and other prescribed individuals regarding the health and care of the patient.
