Job Summary 
 
The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Considered a member of the nursing team and is expected to know, and will be held accountable for, following infection prevention and control policies and Personal Protective Equipment use. The Certified Nursing Assistant (CNA) provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to assigned resident. The CNA assists the resident in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment and also provides range of motion and passive exercises. The CNA completes tasks related to food preparation and dining, while creating a positive dining experience for the resident. The CNA assists residents with meals and snacks, provides water and supplements as appropriate. 
 
The CNA aids providers and nursing staff members with procedures, if needed. Documents resident interactions as needed and the outcomes related to resident care, behavior, activity, and dietary. The CNA communicates resident's changing condition and care related concerns/responses to the charge nurse. The CNA collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational. The CNA provides assistance and documentation with wellness and recreational programs and encourages residents to participate. The CNA may participate in departmental/organization committees and task forces as needed. 
 
Qualifications 
 
High school graduate or general equivalency diploma (GED) preferred, but not required. 
 
Minimum age of 16. Prior clinical or nursing assistant experience in long-term care preferred. 
 
Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. CNA training and certification required and provided by facility. 
 
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department specific required certifications. 
 
When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry. 
 
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor 
 
vehicle report and proof of valid driver&rsquo;s license. Also must be medically qualified to perform the essential driving functions of 
 
this position as per our Sanford Driving Policy per the leaders request.

Job Summary 
 
Provides care and services such as medication administration, healthcare services, activities of daily living assistance and other duties, as assigned, in a caring, safe and efficient manner and is responsible to perform according to location standards and procedures. Assists residents with self-administration of medications. May administer medications with proper training. Assists residents in maintaining proper and clean appearance. Assists residents with bathing and maintaining cleanliness of all body areas. Helps residents with toileting needs. Helps residents with dressing and undressing and maintaining proper clothing. Assists residents in transferring, repositioning and walking. Obtain vital signs accurately and reports changes in resident health status. Adheres to OSHA and CDC guidelines to minimize infections by following aseptic cleaning techniques, including those that apply to blood borne pathogens. Demonstrates safe and proper techniques for chemical use, and follows SDS guidelines for any exposure. 
 
Assists with preparing and serving nutritious, appealing meals and snacks to residents following orders, standards and regulations. Folds, hangs and distributes clean linens and personal clothing. Prepares and launders soiled linen / clothing. Cleans residents' rooms and bathrooms, work areas and other public areas. Takes part in community involvement in location and works with volunteers. 
 
Qualifications 
 
High School Diploma or equivalent preferred. High School Diploma or GED may be required based on working state. 
 
If working at a facility in which a lift is utilized to move residents, must meet the minimum state age requirement. 
 
Must complete and maintain assigned medication administration training program and competencies per state guidelines.

The nursing department provides care 24 hours/day; any of those hours may be an expectation of employment specific to the needs of the department, possibly with varying shifts. These needs change dependent upon staffing availability and resident census. Will be required to work rotating shifts, weekends, and holidays. Specific shift assignments will be made based on nursing area and by location. Committee meeting attendance may be expected at times. When students are providing resident care, treatment, and/or services as part of their training, any/all employees may be asked to provide direction to the students.

Job Summary 
 
The Long-Term Care (LTC) Nursing Assistant (NA) Trainee is enrolled in a learning experience designed to develop the confidence and competence to serve as a caregiver that provides personal care to clients. Under the supervision of a licensed nurse, the trainee, in partnership with a preceptor, provides holistic client-centered care that promotes independence and client rights in a safe, caring, and efficient manner. The trainee is required to work within the role of a non-certified nursing assistant. The trainee is enrolled in a formal nursing assistant program and is required to fulfill all training requirements in the designated timeframe. The Trainee is not allowed to perform any service(s) unless they have been trained and found to be proficient by the instructor. The program is delivered in hybrid format which includes an online curriculum and hands on skills development. While in the Nursing Assistant Training Program, the trainee serves as a member of the nursing team and will be held accountable to know and follow location-specific standards for infection prevention and control, personal protective equipment use, safety, and privacy. The trainee demonstrates the ability to follow written and verbal instructions. This role requires an ability to do detailed work correctly, handle stress, demonstrate critical thinking, and prioritize responsibilities. The trainee works with clients and their families to ensure their physical, cognitive, spiritual, emotional, and social needs are met.
