: The Intake Manager oversees the intake and clinical administrative support teams. This role serves as a vital link between the Director of Patient Care, Director of Marketing and Business Development, and external stakeholders. The Intake Manager is responsible for implementing policies and procedures that align with organizational goals, while providing strategic oversight of referral processes to ensure timely and appropriate patient access to care. Strong leadership, communication, and organizational skills are essential for success in this role.

Qualifications:
<ul><li>Previous supervisory experience</li><li>
Current Minnesota LPN or RN licensure</li><li>
Associate or Bachelor&rsquo;s degree</li><li>
Strong knowledge of hospice regulations, clinical documentation standards, and compliance requirements</li><li>
Must have the ability to direct, supervise, and lead in a professional manner</li><li>
Strong organizational and problem-solving skills</li><li>
Minimum of 3 years&rsquo; hospice experience</li><li>
Excellent written and verbal communications skills</li><li>
Excellent with relationship development internally and externally</li><li>
Ability to think critically</li><li>
Ability to network within the referral communities</li></ul>
Job Duties:<ul><li>
Supervise, mentor and develop direct reports</li><li>
Manage patient records, ensuring compliance with hospice regulations</li><li>
Referral management and oversight</li><li>
Billing
</li><li>Participate in monthly collection call with billing department</li><li>
Supply management</li><li>
Facilitate morning intake call</li><li>
Collaborate with all departments</li><li>
Assist in the recruiting, selecting, hiring and orienting of qualified individuals</li><li>
Manage team staffing needs</li><li>
Supervisor on-call responsibilities</li><li>
Support team through development of programs, goals and objectives consistent with those of the organization&rsquo;s overall strategic plan and initiatives</li><li>
Actively participates in leadership meetings, collaborating and communicating with leadership team</li><li>
Facilitate regularly scheduled team meetings to create and maintain a cohesive team approach in providing excellent end of life care</li><li>
Responsible for building and maintaining employee morale, motivating employees to reach their full potential including professional development
</li><li>Perform clinical administrative tasks as assigned.</li><li>
Uses available resources and data to guide excellent patient care</li><li>
Assist in budget process and staffing plans</li><li>
Support and participate in quality improvement</li><li>
Identify and act upon improvement opportunities for the overall success of Grace Hospice</li><li>
