<div>Must live around the Warrendale, PA area to be considered for this position.
Compensation: $27.00 - $29.00.
The base compensation range for this role is fixed, with a maximum cap of $29.00. We want to be transparent about this as we continue discussions.
Hour: Monday - Friday, 8AM - 5PM,

The Clinical Team Support Coordinator is responsible for supporting assigned clinical care teams in the on-going management of patient care with particular emphasis on monitoring the attainment of episode goals, clinical documentation, workflow, missed visits, among other items to ensure the highest quality clinical outcomes for the agency&rsquo;s patients.
Clinical Team Support Coordinator Responsibilities:<ul><li>
Provide clinical support for assigned care teams.</li><li>
Update Clinical Record on an ongoing basis as relates to communications, orders, re-hospitalizations, conflicts, etc.</li><li>
Evaluate and address customer concerns, including documentation and resolution of same.</li><li>
Participates in staff meeting agenda and in-services as directed.</li><li>
Enter related home health coordination notes into HCHB and Salesforce.</li><li>
Audits clinical records for completeness on and on-going basis and works with professional clinicians to ensure compliance and comprehensive documentation.</li><li>
Review Visit Time Exception</li><li>
Process and resolve unverified visit</li><li>
Review/Clear Held Claim</li><li>
Review Vital Sign Alert</li><li>
Review and Mail Bill of Rights from Team Cord Notification Task</li><li>
Add/Update Facility</li><li>
Assist in triaging patient visits/needs as applicable</li><li>
Payroll audit corrections</li><li>
Consent reports</li><li>
Incoming patient calls</li></ul><h1 class="jobSectionHeader">
Clinical Team Support Coordinator Qualification Requirements:</h1>

Must hold, in good standing, a license as a Practical Nurse or Registered Nurse issued by the state of practice.</li><li>
Must be empathetic, non-judgmental, tactful, responsible, professional and organized.</li><li>
Proficient in Microsoft Office Suite.</li><li>
A minimum of two (2) years of experience working as an LPN/RN including some medical/surgical experience and/or an acute care or home care setting is preferred.</li></ul><h1 class="jobSectionHeader">
Certificates, Licenses, Registrations</h1>
Must have and maintain in good standing a professional license, certificate, or registration, as applicable.<h1 class="jobSectionHeader">
