As a RN Case </i></b><b><i>Manager</i></b><b><i> You will:</i></b></div><ul><li>
Assess/monitor physical, emotional, and psychological needs of patients</li><li>
Create hospice care plans that align with the patients wishes and goals</li><li>
Direct nursing care: administering medications, treatments, and interventions</li><li>
Provide pain and symptom management</li><li>
Educate and support the patient&rsquo;s family and caregivers</li><li>
Collaborate with interdisciplinary team</li><li>
Maintain accurate and timely documentation</li><li>
Participate in on-call rotation as required by local branch</li></ul><div></div><div><b><i><br>

Qualifications:
<ul><li>Graduate of an accredited nursing school with current licensure to practice in state of operation</li><li>
One year nursing experience required, 3+ years preferred</li><li>
Experience in hospice or a similar setting preferred</li><li>
Knowledge of the hospice philosophy of care</li><li>
Commitment to clinical and documentation excellence</li><li>
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order</li></ul>
