Position Overview
Reporting to the Hospice House Clinical Supervisor, the Registered Nurse provides comprehensive case management for patients admitted to the Hospice House. Coordinates resources to plan, organize, implement and evaluate operating routines necessary to achieve residents&rsquo; well-being and organizational objectives.

Collect accurate and current data in partnership with the patient and family in the following areas:</li><li>
Health histories</li><li>
Physical assessment, nutritional needs</li><li>
Emotional, spiritual and social needs</li><li>

Risk factors that affect health</li><li>
Strengths that maintain and promote safety and comfort.</li><li>
Explain the Hospice services, philosophy and limitations during the admission process and provide ongoing reinforcement. Interpret the Hospice as a service of Granite VNA.</li><li>
Consult with patient/family/interdisciplinary team (IDT) in developing a comprehensive plan of care upon admission.</li><li>
Revise the interdisciplinary plan as goals and objectives are achieved and/or changed every 14 days as needed.</li><li>
Record nursing interventions and patient/family responses in a systematic and concise form and according to program policy and procedures.</li><li>
Supervise Hospice LNAs and LPNs according to agency policy and State and Federal regulations</li><li>
Respond to the changing needs of the patient and families during the care of the patient.</li><li>
Provide input into the bereavement needs of the patient and family upon the patient&rsquo;s admission and at IDT meetings thereafter. Provide appropriate support to patients and caregivers throughout the grief process. Serves as the primary nurse and an advocate for the patient and family to achieve Hospice care goals.</li><li>

Expected Hours of Work
The team member will be scheduled to work based on program and Agency needs. Work assignments include holiday, weekend and/or on-call rotation.
Requirements:
<ul><li>Graduate of an accredited nursing program; BSN preferred</li><li>
Must have a minimum of 2 years acute care nursing experience</li><li>
3 to 4 years of recent experience as an RN in home health or hospice medical/surgical in an acute or skilled long-term care setting; 2 years oncology experience and IV Infusion Therapy experience preferred</li><li>
Certification in Hospice and Palliative Care; preferred</li></ul></div>

Reporting to the Hospice Medical Director under the direction of the Chief Medical Officer, the Granite VNA Physician is responsible for determining patient eligibility for Hospice services. Collaborates with internal and external providers.
Essential Functions

Determines patient eligibility for Hospice service in accordance with program acceptance criteria and the Conditions of Participation.</li><li>
Certifies and re-certifies patient eligibility for the Hospice Program in conjunction with the attending physician on a timely basis.</li><li>
Keeps timely, accurate and complete medical records as required by Granite VNA, comply with guidelines and requirements established by Medicare, Medicaid, and all other applicable payors (e.g., patient care documentation and medical billing processes), and maintains patient confidentiality and privacy in accordance with state and federal law including but not limited to the Health Insurance Portability and Accountability Act (HIPAA). This includes Certification of Terminal Illness and Recertification of Terminal Illness documents.</li><li>
Participates in Granite VNA&rsquo;s Hospice interdisciplinary group (IDG) to provide medical consultation and physician representation support at patient case conferencing meetings as requested.</li><li>
Consults with primary physicians or specialists as needed regarding the patient's medical condition. Assumes responsibility for the general medical needs of Hospice patients to the extent that the attending physician does not meet the needs, serving as attending physician as needed and when requested by the patient or their representative. Works with the attending physician to encourage their fulfilling of their responsibilities.</li><li>
Serves as a liaison between the attending physician and the Hospice Team by obtaining/sharing information about the patient's plan of care on an ongoing basis.</li><li>
Evaluates patient's medical status in the home, outpatient, and inpatient settings based on a visit schedule.</li><li>
Works within the hospice provider team to develop an on-call schedule for management approval.</li><li>
Provides Face-to-Face hospice visits in the community, facilities, or Hospice House as mutually agreed upon.</li><li>
Participates in quality assessment and performance improvement.</li><li>
Works with the interdisciplinary team in establishing, implementing, and reviewing the Hospice plan of care for each patient in keeping with the Conditions of Participation.</li><li>
Understands the regulations and standards related to the Medicare Hospice Conditions of Participation.</li><li>
Actively participates in the Hospital and Respite facility joint committees by providing input to them during scheduled meetings.</li><li>
Attends Patient Care Conferences as requested.</li><li>
Develops/reviews and evaluates medical standards, policies, and procedures for the Hospice Program and Agency as requested.</li><li>
Develops and provides in-service for the Hospice Team as requested by the manager, based on identified learning needs.</li><li>
Provides ongoing community and professional education as it relates to the medical community.</li><li>
Fulfills the role of Chief Medical Officer as needed in his/her absence.</li></ul>

Expected Hours of Work

Requirements:

Current NH medical license</li><li>
Board certified in Internal Medicine or Family Practice</li><li>
One to two years of medical physician experience including pain and symptom management experience</li><li>
