About the Job:
The Therapist II is a part of the multidisciplinary team responsible for providing direct clinical services to clients and their families by diagnosing and treating mental and emotional disorders, whether cognitive, affective, or behavioral. Completes Psychosocial Assessments; providing individual, group and family therapy for clients; facilitating psycho-educational training, family meetings, discharge planning; assisting in program development; developing, implementing, and reviewing Treatment Plans; and providing linkage to outside resources.
Roles and Responsibilities:<ul><li>Conducts a psychosocial assessment and develops an initial discharge plan.</li><li>Develops and implements treatment plans for client based on assessment and coordinates any additional services needed, revising as necessary.</li><li>Conducts individual, group or family sessions as appropriate for the treatment plan of the client.</li><li>Applies psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating such diagnosed emotional and behavioral disorders.</li><li>Participates in multi-disciplinary treatment planning conferences. Presents appropriate client information to the treatment team. Recommends effective treatment interventions.</li><li>Maintains a caseload as assigned.</li><li>Develops and implements special client groups or program modules.</li><li>Timely prepares progress notes and treatment updates in the medical record. Maintains all clinical documentation in accordance with regulatory and accrediting standards.</li><li>Ensures each assigned client has a structured and clinically appropriate post care plan. Acts as Liaison to referral sources and support of continuation of care.</li><li>Provides crisis intervention to clients and families in acute distress.</li><li>Provides introduction and initial coordination of wide range of community resources: financial, housing, legal, social/recreational, occupational, spiritual and support groups.</li><li>Provides case management, discharge planning or after-care planning according to treatment plan.</li><li>May assist in educating and training staff on behavior management or therapeutic crisis intervention.</li><li>May coordinate various clinical and psychosocial or educational activities with client and their families.</li></ul><ul><li>Master's Degree in Social Work, Counseling or Psychology required.</li><li>At least two years of experience as a therapist with one or more years&rsquo; experience with the specific population of the facility required.</li></ul>

About the Job:
PURPOSE STATEMENT: 
 
The Licensed Practical/Vocational Nurse works under the direction of a qualified registered professional nurse. Participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the department and within the parameters of the licensing board. Communicates and works collaboratively with members of the multidisciplinary team i.e., medical staff, administration, clinicians, dietitians, and milieu personnel. Takes reasonable care and precautions to ensure high client safety standards and a therapeutic environment exist.
Roles and Responsibilities:
ESSENTIAL FUNCTIONS:<ul><li>
Provides direct and individualized nursing care to an assigned group of clients under the supervision of a registered professional nurse, while always maintaining the assurance of client safety and following unit and facility policies and procedures. Complies with all company safety policies and practices.</li><li>
Assists in relevant health and psychiatric teaching for client and family. Serves as a role model for clients and peers.</li><li>
Shares responsibility for the planning, implementation, and evaluation of nursing care plans and nursing care.</li><li>
Effectively communicates with the medical staff, receives verbal/written orders, and takes care to minimize abbreviation or other errors. Participates in team conferences, sharing knowledge, information, and ideas among team members.</li><li>
Prepares, administers, controls and accurately records medications in accordance with the policy regarding the administration of medications by a licensed practical nurse, with thorough documentation regarding administration effect and/or adverse effects.</li><li>
Observes, documents, and communicates with/reports to the R.N. in charge/supervisor/DON any clinical, physical, unit or personnel, or other issues which could alter client care. Reports status changes and concerns to registered nurses and providers. Communicates with all departments as needed to ensure that total quality client care is delivered.</li><li>
Consults, follows, and documents with sufficient detail nursing care according to facility policy and procedures. Records client symptoms, responses, and progress concisely, accurately, and completely.</li><li>
Safely and accurately performs vital signs, electrocardiograms, phlebotomy/labs, and diabetic/glucose monitoring. Interprets results and takes appropriate nursing action.</li></ul>
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:<ul><li>
Graduation from an approved Licensed Practical/Vocational Nursing Program required.</li><li>
One or more years' nursing experience within clinical area of service provided by facility preferred.</li><li>
Knowledge of and experience with psychotropic medications and medication administration procedures.</li><li>
Ability to apply nursing standards and practice within a recovery setting.</li></ul>
LICENSES/DESIGNATIONS/CERTIFICATIONS:<ul><li>
Valid current registered practical/vocational nursing license from the state or from a nurse compact state, where applicable.</li><li>
CPR certification and de-escalation training required (training available upon hire and offered by facility).</li><li>
First aid may be required based on state or facility requirements.</li></ul>
WORK LOCATION:
This position is onsite and is not a remote position.
SUPERVISORY REQUIREMENTS:
This position is an Individual Contributor.
