We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

<h2 class="jobSectionHeader">Your Impact as a Physical Therapist (PT)</h2>
<ul>
<li>Consult with patients to assess their physical condition and functional limitations
</li>
<li>Evaluate and interpret patient assessments and test results
</li>
<li>Develop individualized treatment plans using a variety of therapy techniques
</li>
<li>Create personalized rehabilitation programs tailored to each patient&rsquo;s goals
</li>
<li>Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
</li>
<li>Ensure all treatments are delivered in accordance with the established plan of care
</li>
<li>Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
</li>
<li>Provide clinical support and education to patients, family members, and caregivers
</li>
<li>Collaborate with interdisciplinary team members to ensure high-quality patient care
</li>
<li>Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
</li>
<li>Comply with all applicable rules, regulations, and procedures
</li>
<li>Attend required meetings as designated by the Director of Rehab
</li>
</ul><h2 class="jobSectionHeader">What Makes You a Great Fit</h2>
<ul>
<li>Active and valid Texas Physical Therapist license
</li>
<li>In good standing with all regulatory agencies and licensing boards
</li>
<li>Skilled Nursing Facility (SNF) or long-term care experience preferred
</li>
<li>Current knowledge of physical therapy treatment practices
</li>
<li>Experience with Net Health Electronic Documentation System preferred
</li>
<li>Ability to manage patients with varying personalities and needs
</li>
<li>Strong communication, collaboration, and clinical decision-making skills
</li>
