Duties and Responsibilities</b></h2><ul><li>
Coordinate with patients, families, and caregivers to facilitate home health service setup during weekends.</li><li>
Handle telecommunications efficiently by answering calls and responding to emails pertaining to home health care inquiries.</li><li>
Manage scheduling of healthcare professionals and adjust rosters based on patient needs and staff availability.</li><li>
Ensure all intake forms are accurately completed and processed in the organization's patient management systems.</li><li>
Collaborate with clinical staff to inform them of new intakes and any specific patient needs or requests.</li><li>
Conduct initial screenings of patient needs and align those with the available resources and healthcare solutions.</li><li>
Maintain complete confidentiality of all patient information, adhering to HIPAA regulations and company policies.</li><li>
Provide updates and report any significant changes in patient handling or scheduling patterns to supervisors.</li><li>
Study and understand healthcare service plans and possibly suggest necessary adjustments or variations.</li><li>
Participate in weekly team meetings to discuss scheduling challenges and opportunities for improvement.</li><li>
Use of technology in maintaining efficient and effective communication across all relevant platforms within the company.</li><li>

Requirements</b></h2><ul><li>
Proven experience in intake coordination, scheduling, or relevant administrative positions within healthcare.</li><li>
High school diploma or equivalent required; additional certification in healthcare administration preferred.</li><li>
