Direct Home Care - Registered Nurse Case Manager, Personal Care Aide Support
 
Health Services of Northern New York is seeking a Registered Nurse Case Manager to support Personal Care Aide activities in St. Lawrence County. This position will be responsible for overseeing aide staff working in patient homes as well as performing admission and discharge services for personal care aide patients.
 
An ideal candidate will have excellent communication skills and be comfortable traveling to and working one-on-one with patients and their families in their home. Additionally, the candidate will provide support and encouragement to our aide staff to help them succeed.
 
This position will require travel to and from patient homes throughout St. Lawrence County. Additional travel may be required at times. This is a full-time position Monday – Friday with a Minimum salary of $66,000/yr.

Registered Nurse Case Manager - HSNNY

Direct Home Care - Case Manager
 Health Services of Northern New York is seeking a Registered Nurse Case Manager to support the southern half of St. Lawrence County.
 
This position will be responsible for the management of client care in the home setting. The Registered Nurse Case Manager will perform nursing assessments, coordinate care with physicians, and have oversite of additional discipline involvement with each client. Additional discipline involvement could include Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker and Home Health Aides.
 This position will require travel to and from patient homes throughout their specified region of St. Lawrence County.
 An ideal candidate will have excellent communication skills and be comfortable traveling to and working one-on-one with patients in their homes.

RN - Case Manager - Southern St. Lawrence County - HSNNY

Direct Home Care - Private Duty Nursing/LPN
 Health Services of Northern New York is seeking an LPN for Private Duty Nursing.
 This position would be responsible for Complete Care of a quadriplegic person, to include catheterization, tracheostomy care, use of ventilator at night only. Client is self-directing.
 
A successful candidate must have strong communication skills and be comfortable working one on one with a patient in their home.
