Job Description: We are seeking a skilled and compassionate Nurse Practitioner to join our medical practice in OH. Our focus is on providing high-quality care to patients in skilled nursing facilities and assisted-living facilities mainly with elderly patients.
Position Details:<ul><li>Part-time role: 2-4 shifts per month</li><li>Paid per diem</li><li>Opportunity to make a meaningful impact on patient care</li></ul>
Responsibilities:<ul><li>Conduct comprehensive patient assessments and physical examinations.</li><li>Develop and implement treatment plans in collaboration with supervising physicians.
</li><li>Perform diagnostic procedures and interpret laboratory results.</li><li>Provide patient education on health management and disease prevention.</li><li>Administer medications and treatments as prescribed.</li><li>Document patient information accurately in electronic medical records (EMR).</li><li>Assist in the management of patients in nursing homes or long-term care facilities.</li><li>Stay current with advancements in medical practices and technologies.</li></ul> 
Ideal Candidate:<ul><li>Holds current certification and licensure as a NP/PA in OH.</li><li>Experience in post-acute care or skilled nursing facilities (preferred but not required)</li><li>Strong clinical, communication, and interpersonal skills</li><li>Experience with telehealth platforms and remote patient monitoring.</li><li>Experience in Geriatrics (preferred)</li><li>Ability to work effectively within a team environment while demonstrating excellent communication skills.</li></ul>

Job Description
We are seeking a skilled and compassionate Physician (MD/DO) to support our medical practice in California. Our focus is on providing high-quality care to patients in skilled nursing facilities and assisted living communities, primarily serving elderly patients.
Position Details<ul><li>Per diem role: approximately 2&ndash;4 shifts per month</li><li>Independent contractor (1099)</li><li>Flexible scheduling based on clinical needs</li><li>Opportunity to make a meaningful impact on patient care</li></ul>
Responsibilities<ul><li>Provide medical evaluations and clinical oversight for patients in skilled nursing and assisted living facilities</li><li>Review patient histories, diagnostic results, and clinical data to support appropriate care decisions</li><li>Collaborate with nurse practitioners, physician assistants, and clinical staff to coordinate patient care</li><li>Provide medical guidance related to chronic condition management and patient monitoring</li><li>Support care planning and treatment recommendations as appropriate</li><li>Ensure accurate and timely documentation in electronic medical records (EMR)</li><li>Participate in care coordination and communication with facility staff and the care team</li><li>Maintain compliance with applicable clinical, regulatory, and organizational guidelines</li></ul>
Ideal Candidate<ul><li>Active MD or DO license in California</li><li>Experience in post-acute care, geriatrics, or skilled nursing facilities (preferred)</li><li>Strong clinical judgment and communication skills</li><li>Experience with telehealth platforms or remote patient monitoring (preferred)</li><li>Ability to work effectively within a collaborative care team</li></ul>
Compensation
This is a per diem independent contractor (1099) position.
