<div>Position Description
Harlem United is the parent company for Upper Room AIDS Ministry (URAM). URAM provides the healthcare services from the Federally Qualified Health Center located in the heart of Harlem. We provide Primary Care, Specialty Care, Dental Care and Wellness Care to those seeking diagnosis and treatment. All employees of URAM are responsible to perform their job practicing good risk management, quality assurance, and delivering excellent patient experience.
Under the supervision of the Medical Director and in accordance with established policies and procedures, the Physician delivers compassionate, high quality comprehensive primary care to homeless, underserved, chronically ill populations including those with diagnoses of HIV, Hepatitis C, SUD, Diabetes, Heart Disease and other comorbidities.
This role includes 80% direct patient care and 20% administrative time. The schedule is 9 AM to 5 PM (4 days a week) and 11 -7 (one day per week). Phone call only.
Essential Duties and Responsibilities
Essential duties and responsibilities include, but are not limited to, the following:<ul><li>
Provides Primary Care, routine and acute care, for both scheduled, same day and walk in appointments</li><li>
Maintains a professional demeanor, including demonstration of sound moral and ethical character, compassion, adherence to all agency standards for dress and decorum</li><li>
Initiation of treatment and management of chronic illnesses including, but not limited to, HIV infection, asthma, bronchitis, diabetes, lipid disorders, and hypertension</li><li>
Generates appropriate referrals to ancillary and specialty services</li><li>
Maintains documentation according to standards of the agency and regulatory agencies</li><li>
Initiation/ changes in treatment</li><li>
Referrals to specialty services, or social-based services</li><li>
Ensure timely prescription writing, order, progress notes, and referrals</li><li>
Follows prescribing practices according to agency policy and state regulations</li><li>
May administer medication according to the same procedures and protocols applied to the nursing staff</li><li>
Performs routine diagnostic and therapeutic procedures according to accepted practice standards</li><li>
Interpretation of laboratory studies and follow-up on consultation reports</li><li>
Provide health education and counseling during visits</li><li>
If family medicine trained, the physician is expected to care for patients of all ages</li><li>
Documents and locks progress notes within 72 hours of client encounter</li><li>
Documents clinical activities and information</li><li>
Demonstrate proficiency in use of electronic medical record</li><li>
Complies with productivity expectations under the direction of the Medical Director</li><li>
Adheres to regulatory guidelines in regards to compliance, ie, Coding practices.</li><li>
Attends patient care related meetings, as necessary, including but not limited to morning huddles, case conferences.</li><li>
Manages coordination of care for individual clients by working with available resources both internal and external to the agency</li><li>
Provide clinical supervision to Advanced Practice Clinicians (PA and NP) as assigned by the Medical Director</li><li>
Provide phone on-call coverage on a rotating schedule</li><li>
Cover one evening (weekly) or one Saturday clinic (monthly)</li><li>
Participates in assigned shifts with the mobile/street medicine.</li><li>
Participates in CQI activities</li><li>
Other duties as assigned by the Medical Director</li></ul>
Other Responsibilities<ul><li>
Participate in required clinic meetings</li><li>
Attends conferences and community-based activities, including advocacy efforts to eliminate homelessness, HIV and disparities in care as designated by the Medical Director or designee</li><li>
Participates in Collaborative Activities as designated by the medical director</li></ul>
Qualifications/Education/Experience<ul><li>
Medical Doctor licensed to practice in the State of New York</li><li>
Board certified OR board eligible in internal medicine or family medicine</li><li>
Achieve and maintain HIV specialist status (preferred)</li><li>
Accumulate a minimum of 10 specific HIV CME credits throughout the year and a total of 25 credits total per year</li><li>
Possess or obtain within first 90 days of employment a DATA Waiver for Buprenorphine Prescribing authority.</li></ul>

Ideal candidate for this position possesses most or all of the following:<ul><li>
Bilingual preferred (English/Spanish or English/French)</li><li>
Experience in caring for people living with HIV, populations with experiences of homelessness, substance use disorders, and mental illness.</li><li>
Experience in caring for people living with HIV, populations with experiences of homelessness, substance use disorders, and mental illness.</li><li>
Experience in caring for immigrant communities</li><li>
Desire and ability to work as part of an interdisciplinary team dedicated to the delivery of high-quality and compassionate care.</li><li>
Strong problem-solving skills and ability to work independently with in-person supervision.</li><li>
Proficient in use of electronic medical record systems and other technology required for use in the delivery of care</li><li>
Positive interpersonal skills with staff, clients, and others</li><li>
Willingness to work off hours on special projects as reasonably dictated by the medical director</li><li>
The spirit and desire to practice in a community based interdisciplinary mission driven system of care</li></ul>
Expectations<ul><li>
Complete Certification as an HIV Specialist with AAHIVM in the first 12 months of employment</li></ul>
Special Requirements<ul><li>
The employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye/hand coordination. Requires sitting, standing and walking for extensive periods of time. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.</li></ul>
Compensation and Benefits:<ul><li>
Salary Range: $210,000 to $225,000</li><li>
