As a On-Call RN You will:</i></b></div><ul><li><b>
(CHOOSE ONE: If office HAS a Remote Triage team)</b> Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis</li><li><b>
(CHOOSE ONE: If office DOES NOT have Remote Triage) </b>Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis</li><li>
Assess and manage urgent symptoms quickly and compassionately</li><li>
Administer medication or adjust care based on hospice protocols or provider orders</li><li>
Communicate with the physician regarding changes in condition and obtain orders as necessary</li><li>
Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps</li><li>
If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team</li><li>
Complete timely documentation and communication</li></ul><div><b><i>

Qualifications:
<ul><li>Graduate of an accredited nursing school with current licensure to practice in state of operation</li><li>
One year nursing experience required, 3+ years preferred</li><li>
Experience in hospice or a similar setting preferred</li><li>
Knowledge of the hospice philosophy of care</li><li>
Commitment to clinical and documentation excellence</li><li>
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order</li></ul>
