A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home, following physician&rsquo;s orders.</div></div><div></div><div><div><b><br>
Essential Functions</b></div></div><div><ul><li>
Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures.</li><li>
Prepare an assessment of the patient&rsquo;s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.</li><li>
Recommend modification of plan of care to physician, as needed.</li><li>
Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date.</li><li>
Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.</li><li>
Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.).</li><li>
Include and educate the patient and caregiver(s) regarding treatment plan and goals.</li><li>
Attend physical therapy staff meetings and in-service presentations.</li><li>
Participate in case conferences.</li><li>
Participate in the development and periodic revision of the physician&rsquo;s Plan of Treatment.</li><li>
Assure appropriate safety practices and maintenance of equipment.</li><li>
Other duties as assigned.</li></ul></div><div></div><div><div><br>

Education Requirements</b></div></div><div><ul><li>
Graduate of an accredited Physical Therapy program.</li></ul></div><div></div><div><div><b><br>
Qualifications and Requirements</b></div></div><div><ul><li>
Current, valid Physical Therapy license issued by NYS Department of Education.</li><li>
One year of practical experience as a licensed physical therapist.</li></ul></div><div></div><div><div><b><br>
Work Environment</b></div></div><div><div>
The Physical Therapist is primarily in a non-office setting and may be exposed to outdoor conditions.</div></div><div></div><div><div><br>
The working conditions are classified as medium work:</div></div><div><ul><li>
Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.</li></ul></div><div></div><div><div><b><br>
