The Treasure Island Residential Step Down (RSD) services are available to beneficiaries who are stepping
 down from inpatient/residential substance use disorder treatment. Clients must be concurrently in
 treatment, specifically in outpatient (OP), intensive outpatient (IOP), Opioid Treatment Program (OTP), or
 Outpatient (aka Ambulatory) Withdrawal Management (OP-WM) settings. RSD provides a temporary,
 drug and alcohol-free environment to residents that are actively engaged in outpatient treatment for
 medically necessary SUD provided to the client off-site.
 
The On-Call Recovery Coach is responsible for assessing client needs and strengths in support of health
 and recovery. Through individual coaching with program-specific requirements and case management,
 On-Call Recovery Coaches assists clients in navigating systems of care while providing a supportive
 recovery environment. On-Call Recovery Coaches are responsible for maintaining and assessing the safety
 and security of the program and its residents through continued house runs while providing a supportive
 environment. Through effective communication, On-Call Recovery Coaches relay relevant information to
 other members of the interdisciplinary team and perform crisis intervention as needed. On-Call Recovery
 Coaches ensure that the facility environment is safe and welcoming.
 
KEY RESPONSIBILITIES
 
Client Services: 
<ul>
<li>Welcomes and assists clients with navigating throughout the building.</li>
<li>Provides excellent customer service and addresses any immediate needs of patients.</li>
<li>Assists patients with the required paperwork as needed for proper enrollment in services.</li>
<li>Coordinates transportation for patients in residential programs.</li>
</ul>
Individual Responsibilities: 
<ul>
<li>Provide direct services such as intakes, assessment, crisis intervention, and case management as</li>
</ul>
needed.

<li>Responsible for retention and completion of clients, facilitate individual coaching sessions with</li>

each caseload client.

<li>Provide learning experience opportunities and offers support to assist clients in meeting their</li>

long-term recovery goals.

<li>Proactively link clients to both internal and external resources based on their recovery needs and</li>

follows up on the progress/status.

<li>Perform house runs to ensure and maintain accountability of all clients and the safety and security</li>

of the facility.

<li>Perform crisis intervention and communicate with treatment team as unforeseen situations arise.</li>
<li>Document client updates and incidents in the facility log daily, perform CPR and First Aid including</li>
</ul>
administering Narcan as needed, and work weekends and holidays as needed.
 
Treatment Setting Responsibilities:

<li>Facilitate community meetings and ensure compliance with facility rules.</li>
<li>Perform crisis intervention and communicate with team as unforeseen situations arise.</li>
<li>Document all client updates and facility incidents daily in the facility log.</li>
<li>Perform periodic house runs to ensure and maintain the safety and security of the facility.</li>
<li>When assigned at the Health and Wellness office, the Recovery Coach will document and</li>
</ul>
accurately distribute client money, ensure client medications are securely stored and properly
 accounted for and holds facility keys and floater cellular phones.
 <ul>
<li>As needed, accompanying clients to offsite appointments related to their recovery.</li>
<li>Attending all required training and meetings as assigned by facility manager.</li>
<li>Plan and facilitate client celebrations and special events.</li>
<li>Recovery Coach must be available to work weekends and holidays as needed.</li>
</ul>
Documentation Responsibilities: 
<ul>
<li>Collaborate with each client and other available internal and external resources to develop</li>
</ul>
transition plans, progress notes and appropriate updates in support of the health and recovery
 needs of the client.
 <ul>
<li>Properly document all individual encounters and complete the discharge paperwork/process and</li>
</ul>
required agency assessments in timely manner.
 <ul>
<li>Maintains accurate records by data entering documentation into various electronic systems for</li>
</ul>
all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and
 external evaluating requirements.
 <ul>
<li>Completes accountability report on a weekly basis and collaborates with treatment providers to</li>
</ul>
assist clients achieve program completion.
 Safety Responsibilities

QUALIFICATIONS
 Education, Certification, and Experience

The Chef De Partie guides a team of staff and clients, executes food preparation, organizes catering in-house operations, ordering, and transporting/budgetary objectives for the agency. Additionally, the Chef De Partie ensures the safety of food handling practices, proper sanitation, utilization of the First In, First Out (FIFO) method and keeps supplies stocked. JOB SUMMARY
 HealthRIGHT 360, a family of programs, is located across 13 different counties in California. HealthRIGHT
 360 provides an array of medical and behavioral health services to some of the most vulnerable
 populations, ranging from primary care, mental health services, detox, substance use disorder treatment
 in both residential and outpatient settings, and transitional services for the criminal justice population.
 
The Specialized Treatment for Optimized Programming (STOP) Area Six Program connects California
 Department of Corrections and Rehabilitation inmates and parolees to comprehensive, evidence-based
 programming and services during their transition into the community, with priority given to those
 participants who are within their first year of release and who have been assessed to as a moderate to
 high risk to reoffend. Area Six includes San Diego, Orange, and Imperial counties. STOP subcontracts with
 detoxification, licensed residential treatment programs, outpatient programs, professional services, and
 reentry and recovery housing throughout the program area to assist participants with reentry and
 recovery resources.
 
The Case Manager works with a multidisciplinary team to provide casework services to clients, handle
 intake and initial placement in community-based services, transportation, case plan, case management,
 and interface with Division of Adult Parole Operations (DAPO) to assist clients in navigating the needed
 services to reach health and recovery. Case Manager I ensures that the quality of services delivered by
 the community-based providers (CBP) meet the standards set forth by the California Department of
 Corrections and Rehabilitation (CDCR) and HealthRIGHT 360.
 
KEY RESPONSIBILITIES
 
Case Management Responsibilities: 
<ul>
<li>Maintains an assigned caseload of up to fifty (50) CDCR clients, or more depending on needs.</li>
<li>Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals.</li>
<li>Develops Case Management Plans (CMP) for clients.</li>
<li>Develops and monitors clients’ progress relative to their CMP through all of phases of the program and links them</li>

Prototypes CHS-SUD Substance Treatment and Reentry Transition (START) provides direct in-custody SUD
 counseling, group therapy, and case management services to up to 150 women located at the Century
 Regional Detention Center (Lynwood County Jail). START services are evidence-based and culturally
 appropriate that utilize cognitive behavioral therapies and motivational interviewing approaches to treat
 addiction.
 
The Care Coordinator II is an integral part of an interdisciplinary team that supports our clients in their
 treatment and recovery by coordinating services, providing clinical assistance, and ensuring accurate
 documentation. The Care Coordinator II will develop individualized treatment plans and will help our
 clients meet treatment goals, manage crisis, assist with public benefits, and facilitate access to educational
 and community resources. Additionally, this role will advocate for our clients and ensure that they have
 the resources needed for successful reentry and long-term recovery.
 
KEY RESPONSIBILITIES
 Client Services & Coordination

 Men’s Residential and Detox or 890 Residential Services is a program of HealthRIGHT 360 that serves male-identifying clients. This program has approximately 105 beds and provides residential treatment for substance use disorder (SUD) for up to 90 days. Clients are supported by a team of behavioral health professionals who provide counseling and education for substance abuse disorders, life skills, as well as individual and group therapy.
 The On-Call Therapist works collaboratively with a team consisting of Substance Use Disorder (SUD) Counselors and SUD Supervisors to provide valuable insight, direction, and oversight of Drug Medi-Cal (DMC) services. They also participate in a collaborative care process for clients, which includes providing individual and group therapy. Additionally, they are responsible for providing safe, supportive, and confidential counseling; maintaining a culturally competent stance; and always creating and maintaining a trauma informed program environment.
 
KEY RESPONSIBILITIES

<li>Performs individual psychotherapy services in a residential setting.</li>
<li>Provides counseling, support and psycho-education for individuals with co-occurring disorders that include screening clients for appropriate levels of services, assessments, treatment planning, case management, and referrals.</li>
<li>Maintains confidentiality while working as part of a treatment team and adhering with program norms.</li>
<li>Participates in case conferences, treatment planning and clinical discussions to provide holistic care.</li>
<li>Communicates with prescribing psychiatrists regarding psychotropic medication compliance, side effects, follow up appointments.</li>
</ul>
Treatment Setting Responsibilities: 
<ul>
<li>Facilitates/co-facilitates process and psycho-educational groups for chronically mentally ill adults with co-morbid substance abuse diagnoses.</li>
<li>Assists Intake Department with initial psychological assessments for individuals seeking admission to program.</li>
<li>Assists clients and their care coordinators with referrals and other linkages to community resources.</li>
<li>Performs floor duties in the milieu that include (but are not limited to) assisting clients in coordinating outside appointments and transportation, overseeing client participation in program activities, monitoring medications compliance, and mediating client conflict when it arises.</li>
<li>Actively participate with the treatment team to continuously assess and refine clinical programming efforts.</li>
<li>Manages client risk and crisis intervention.</li>
<li>On call after hours pager rotation as well as rotating ‘Therapist of the Day’ responsibilities during regular business hours.</li>
<li>Attends staff and agency meetings.</li>
<li>Participates in agency trainings as required.</li>
<li>Complies with Board requirements for license registration/renewal.</li>
</ul>
Documentation Responsibilities: 
<ul>
<li>Conducts assessments, completes ANSAs, develops treatment plans, completes release and consent forms, billing and reporting paperwork in hard copy and online in various electronic systems.</li>
<li>Completes Medicare and Medi-Cal documentation and billing.</li>
<li>Documents individual and group services delivered in various electronic systems in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.</li>
</ul>
Other Responsibilities: 
<ul>
<li>Conduct mental health assessments and follow up with each client and treatment counselor.</li>
<li>Identify treatment needs, recommend treatment plans based on assessment, and work to ensure clients are able meet all requirements.</li>
<li>Provide diagnostic feedback and assessment information to facilitate accurate level step up or step-down services to support client’s goals.</li>
<li>Monitor client conduct, record client activity, and inform colleagues of potential challenges in treatment episodes.</li>
<li>Provide crisis assessment and intervention as needed.</li>
<li>Maintain appropriate/ethical boundaries with all clients and families served.</li>
<li>Facilitate timely access to mental health services on site and within the community.</li>
<li>Serve as the point of contact and coordinate referrals and follow-ups with health agencies.</li>
<li>Coordinate discharge planning with client’s care coordinator.</li>
<li>Facilitate psychoeducational groups on a variety of core curriculum topics including CBT, Seeking Safety, DBT and Coping with Stress Triggers.</li>
<li>Monitor and maintain EMR reports to prevent and to correct any errors.</li>
<li>Maintain compliance with due dates for all clinical documentation including assessment, treatment plan, treatment plan updates, re-assessments, discharges, outcome measures, and other clinical documents in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.</li>
</ul>
And perform other duties as assigned.
 
QUALIFICATIONS
 Education, Experience, and Credentials
 <ul>
<li>Master’s degree in Psychology, Counseling, Social Work, Marriage and Family Therapy, or related field from an accredited graduate school.</li>
<li>Registration (AMSW, AMFT, APCC) from a regionally accredited graduate school and registration with the Board of Behavioral Sciences (BBS).</li>
<li>First Aid and CPR Certified within 30 days of employment.</li>
<li>Possession of valid California driver's license.</li>
<li>Experience with Drug Medi-Cal operations.</li>
<li>Experience with mental health assessment and diagnosis.</li>
<li>Experience establishing supportive relationships and working with clients from culturally and linguistically diverse backgrounds.</li>
<li>Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.</li>
</ul>
Experience in behavioral health and residential settings required.
 
Knowledge and Skills

<li>Strong proficiency with Microsoft Office applications and internet applications.</li>
<li>Ability to work within a frequently changing project scope while maintaining overall direction and structured priorities.</li>
<li>Excellent interpersonal, written, and verbal communication skills.</li>
<li>Ability to enter data into various electronic systems while maintaining integrity and accuracy of data.</li>
<li>Ability to handle sensitive information in a confidential manner.</li>
<li>Flexibility to alter work schedule to accommodate client availability.</li>
<li>Must complete background check and livescan clearance.</li>
