<div>At Healthcare in Action, we serve patients experiencing homelessness through an innovative &ldquo;street medicine&rdquo; approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.
Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.
Healthcare in Action was established in 2021 as a 501 (c)3 organization and associated medical group. Utilizing mobile clinics staffed by healthcare professionals, community health workers, and social workers, our teams travel directly to unhoused individuals living in communities across California, providing services where they reside &ndash; including tents, cars, shelters and temporary housing. In meeting patients where they are and supporting their goals, HIA team members build trust &ndash; a critical component that can lead to meaningful and positive changes in health and housing.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.
The Primary Care Lead Physician will provide clinical leadership and oversight for Advanced-Practice Practitioners (NPs and/or PAs). In addition, the Primary Care Lead Physician will perform direct patient care to a panel of patients in the Los Angeles, CA area .
Responsibilities include:
<ul><li>Provide direct care to patients experiencing homelessness.
<ul><li>Triage, assess, screen, diagnose and treat patients either in-person or via telephone or video visits as needed.
</li><li>Provide care at patient&rsquo;s site of residence which may include the streets, tents, shelter, interim housing sites or mobile unit.
</li><li>Review laboratory, radiologic or other diagnostic tests.
</li><li>Prescribe medications.
</li><li>Document in the electronic medical record.
</li><li>Perform medical procedures as needed.
</li><li>Collaborate with a multi-disciplinary team of social services and clinical personnel to develop a comprehensive care plan.
</li></ul></li><li>Supervise Advanced Practice Practitioners (APPs) including Physician Assistants and Nurse Practitioners in managing complex patients through direct field observations of team members, seeing members in tandem with the team, and providing remote guidance.
<ul><li>Be available to provide in-person, telephone or virtual assistance as needed during work hours.
</li><li>Participate in Quality Care and Peer Review meetings.
</li><li>Create schedules, approve time-off requests and arrange coverage for APPs as needed to ensure patient care needs are met.
</li><li>Mentor, help educate, and provide feedback to APPs.
</li><li>Participate in patient and in-service training for support personnel.
</li></ul></li><li>Coordinate with other primary care physicians, hospital personnel, specialists, and social services staff to coordinate care for post-acute care patients.
</li><li>Participate in community stakeholder meetings to develop and maintain relationships with community-based organizations with mutual patient-care goals.
</li><li>Other projects and duties that promote patient care and Healthcare in Action&rsquo;s overall mission as assigned.
</li><li>Knowledge of the treatment of complex chronic diseases, social determinants of health, psychiatric illnesses and substance use disorder diagnosis and management.
</li><li>Flexibility to work in a start-up with changing goals/metrics.
</li><li>Ability to identify team needs and coordinate with other resources/programs appropriately.
</li><li>Knowledge of HCC and ICD-10 coding and documentation.
</li><li>Knowledge of wound care and management.
</li><li>Strong interpersonal skills and change agent mindset needed to build relationships with healthcare team and patients.
</li><li>Cultural competence and sensitivity required, multilingual skills preferred, especially Spanish.
</li><li>Knowledge of and ability to maintain HIPAA requirements.
</li><li>Adaptable and open to change and problem-solving.
</li><li>Knowledge of and/or ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and open to new technology
</li><li>Proof of Valid Driver&rsquo;s License
</li><li>Maintain State Liability Insurance
</li><li>Have access to a Vehicle.
</li></ul>Qualifications and experience:
<ul><li>MD or DO, Required
</li><li>Family Medicine, Internal Medicine, Emergency Medicine trained, Preferred
</li><li>Unrestricted California State Medical License, Required
</li><li>Board Certification in one of ABMS categories, Required
</li><li>1+ years of related experience as a physician, preferably in a primary care, urgent or emergency care, or hospital setting
</li><li>Mentorship or direct people supervision strongly preferred.
</li></ul>Tuberculosis Screening Policy
