</div><p><b>Licensed Practical Nurse</b></p><p><b>
Position Summary:</b></p><p>
The Licensed Practical Nurse (LPN) provides community-based nursing support to a Family Nurse Practitioner (FNP) serving adults with mental health needs. This role focuses on delivering compassionate, high-quality care in home and in the community. The LPN assists with care coordination, medication support, health education, and monitoring of clients with behavioral health conditions, helping promote stability, wellness, and improved quality of life.</p><p></p><div><b><br>
Job Responsibilities:</b></div><div><ul><li>
Maintain total confidentiality and adhere to HIPAA law, CARF accreditation standards and Maryland State Regulations</li><li>
Assist the Family Nurse Practitioner with client assessments, follow-up visits, and care plan implementation</li><li>
Perform basic nursing tasks including vital signs, symptom monitoring, wound care (if applicable), and medication administration under supervision and within scope</li><li>
Conduct community-based visits to monitor client health status, medication compliance, and psychosocial needs</li><li>
Using electronic medical records (EMR), input data and following through seeing services to their completion</li><li>
Support mental health interventions by observing, documenting, and reporting behavioral changes and risk concerns to the FNP or clinical team</li><li>
Provide medication education and reinforce treatment plans developed by the FNP</li><li>
Assist the FNP with collecting data, updating treatment plans, and completing required forms or reports</li><li>
Administer prescribed medications, including injections under the supervision of the prescriber or register nurse</li><li>
Collaborate with the FNP, therapists, case managers, social workers, and external providers.</li><li>
Take part in all agency training programs as requested</li></ul></div><div></div><div><b><br>
Qualifications, Educations and Experience:</b></div><div><ul><li>
Must possess an active and unrestricted license as a Practical Nurse</li><li>
Minimum 2 year of nursing experience; behavioral health, community health, or home care experience preferred</li><li>
Valid driver&rsquo;s license with a safe driving record</li><li>
Strong communication, documentation, and problem-solving skills</li><li>
Demonstrated ability to work collaboratively in a teamwork environment</li><li>
Current CPR and First Aid certification preferred</li></ul></div><div></div><p><b><i><br>

Position Summary:</b></p><p>
The Licensed Practical Nurse is responsible for assisting providers with office visits and provide skilled nursing services as scheduled and document information in the organizations EMR. This position will play a vital role in patient quality care.</p><p></p><div><b><br>
Job Responsibilities:</b></div><div><ul><li>
Maintain total confidentiality and adhere to HIPAA law, CARF accreditation standards and Maryland State Regulations.</li><li>
Using the electronic medical records (EMR), input data and following through seeing services to their completion.</li><li>
Take part in all agency training programs as requested.</li><li>
Complete specialty referrals on behalf of providers: collaborate with specialty offices to ensure continuity of care and receipt of specialty services.</li><li>
Complete prior authorizations for services and medications; collaborate with pharmacy, pharmaceutical, and service vendors to ensure continuity of care.</li><li>
Administer prescribed medications, including injections under the supervision of the prescriber or register nurse.</li><li>
Collect and process lab specimens</li><li>
Complete all regulatory documentation of client treatment and share information in a timely</li><li>
manner with other professionals working with client, both with the staff and, with client permission, outside agencies.</li><li>
Assist providers with office visits and provide skilled nursing services as scheduled and document information in the organizations EMR.</li><li>
Escort patients to exam rooms, interviews patients, measure, and document vital signs.</li><li>
Monitor the nurse line to triage and respond to client needs through collaboration with the care team, which includes pharmacy coordination.</li><li>
Keep exam rooms stocked with adequate medical supplies, maintain instruments, prepare sterilization as required; Properly disposes of contaminated supplies</li><li>
Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls</li><li>
 Collaborate with rehabilitation/treatment plans and staff in serving the client&rsquo;s needs.</li><li>
Provide health promotion and training as requested.</li><li>
Performs other duties as assigned by supervisor.</li></ul></div><div></div><div><b><br>
Qualifications, Educations and Experience:</b></div><div><ul><li>
Must possess a current license as a Practical Nurse from the Maryland Board of Nursing.</li><li>
Two years of nursing experience, preferably working with those who have mental illness.</li><li>
Demonstrated ability to work collaboratively in a teamwork environment.</li><li>
Highly developed organizational and communication skills.</li><li>
Availability for work including some evenings as needed.</li><li>
Current CPR certification.</li><li>
Privileged to carry out all duties, including client treatment coordination.</li></ul></div><div></div><p><b><i><br>
