HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.REQUIREMENTS:<ul><li>A Master&rsquo;s Degree from an accredited college or university, specializing in psychology, social work, counseling, or other related human services field</li><li>Current, appropriate licensure/certification issued by the State of Michigan</li><li>Possess a CAADC, CADC or file for a development plan within 30 days of employment</li><li>One year providing direct treatment services to adults in a behavioral health inpatient or outpatient setting</li><li>Demonstrate knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a substance abusing adult population</li></ul>RESPONSIBILITIES (summary):<ul><li>The SUD Therapist will also work with drug/sobriety court clients.</li></ul>Intake<ul><li>Facilitate new patient orientation group, as assigned, and provide accurate information regarding clinic and corporate CMH services.</li><li>Complete assigned new intake comprehensive bio-psychosocial assessment.</li><li>Formulate preliminary DSM V Diagnosis.</li><li>Identify issues that may exist regarding safety (personal and environmental), natural support, and community involvement.</li><li>Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV, education, and vocational training.</li><li>Identify Targeted Case Management needs for CMH-eligible patients and complete referral recommendation.</li><li>Obtain releases of information for coordination of care.</li><li>Demonstrate knowledge of patient payor source.</li></ul>Treatment Planning and Progress Reviews<ul><li>Complete an individualized Treatment Plan for each patient.</li><li>Utilize the patient&rsquo;s Person-Centered Questionnaire in development of treatment plan and support person participation.</li><li>Complete Relapse Prevention Plan within 30 days of admission.</li><li>Treatment Plan problem areas, goals, and objectives address the patient&rsquo;s DSM five-axis diagnosis and areas identified on the bio-psychosocial assessment.</li><li>Demonstrate use of natural supports in Treatment Plan.</li><li>Complete Treatment Plan Reviews.</li><li>Update Treatment Plan via a TPR following a change in clinical status</li></ul>Discharge Process<ul><li>Document on Treatment Plan criteria and anticipated date for planned discharge.</li><li>Update changed discharge criteria on the TPR.</li><li>Discharge patients with no contact for 45 days.</li><li>Provide written notification to Medicaid recipients of intent to terminate services.</li><li>Complete Discharge Summary within 21 days of discharge date.</li><li>Complete a Continuing Care Plan for each patient discharge at the last session for planned discharges and within seven days for unplanned discharges.</li></ul>Ongoing<ul><li>Provide individual supportive, solution-focused psychotherapy not less than once every 30 days for each patient on caseload.</li><li>Provide group therapy, as assigned, demonstrating utilization of supportive and/or psycho-educational approach.</li><li>Start and stop therapy, collateral, and consultation sessions as scheduled, unless crisis intervention services require otherwise.</li><li>Initiate contact, by phone or in writing, to non-compliant patients within 24 hours of a missed appointment.</li><li>Report medication issues, including non-compliance and adverse side effects, to clinic psychiatrist and/or CSS or Program Manager immediately.</li><li>Monitor substance use via Urine Drug Screens, as indicated.</li></ul>Pay is based upon years of related experience.
