Helio Health is looking for a <b>Case Manager</b> to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is a<b> </b><b>Full-Time</b> position, and located at our <b>Syracuse Supported Housing program</b>.</div><div></div><div><b><br>
Pay Rate</b>: $20.00-$24.00 per hour.</div><div></div><div><br>
In addition to Helio Health&rsquo;s comprehensive benefits package, Full-Time hires for this position are eligible for a <b>Sign-On Incentive</b> up to <b>$1,500</b>!</div>
Responsibilities:
<ul><li>Maintains a case load and performs primary case work responsibilities including:<ul><li>
Screening, intake, orientation</li><li>
Assessment, evaluation, and intervention (Only performed by Case Managers with QHP)</li><li>
Treatment planning (Only performed by Case Managers with QHP)</li><li>
Referrals</li><li>
Counseling</li><li>
Crisis intervention</li><li>
Patient/resident education</li><li>
Case management</li><li>
Reporting and record keeping</li><li>
Consultation with other professionals</li><li>
Coordination of care with other community providers</li><li>
Discharge planning</li></ul></li><li>
Trains of staff, volunteers, and students/interns</li><li>
Performs Quality Assurance activities<ul><li>
Provides services according to agency policies, procedures, program guidelines, and governmental regulations.</li><li>
Monitors provision of services when assigned.</li><li>
Evaluates programs and identifies strengths/weaknesses.</li><li>
Assists in problem resolution and implementation of supervisor approved program modifications.</li></ul></li><li>
Participates in program administrative and clinical committees as assigned.</li><li>
Represents agency to the community.</li><li>
Other duties as assigned by supervisor.</li></ul>
Qualifications:
<ul><li>Associate's degree in a related human services field required, Bachelor's preferred.</li><li>
Two years of experience working with behavioral health and/ or substance abuse issues.</li><li>
Valid NYS Driver's License preferred.</li></ul><div></div><div><b><br>

Case Manager - Supported Housing Program

To support the Mission of Helio Health, Inc., the <b>CTI Case Manager</b> will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a &ldquo;critical time&rdquo; transitional period in their lives. CTI is designed specifically to enhance the continuity of care during transition from hospital to community care.</div><div></div><div><b><br>
CTI Case Manager:</b></div><div>
Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the <b>Critical Time Intervention Case Manager </b>is a care manager, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual&rsquo;s ability to meet recovery goals. The case manager will work on a <b>Full-Time</b> basis and using a hybrid work model, working in the community throughout <b>Montgomery County</b>.</div><div></div><div><br>
The schedule is <b>Monday-Friday, 8:00am-4:30pm,</b> with <b>one late shift per week</b> (12pm-8:30pm), and <b>some evening on call</b> responsibilities.</div><div></div><div><b><br>
Pay Rate:</b> $23.00-$24.52 per hour.</div><div></div><div><br>

Qualifications:<br>

Bachelor&rsquo;s degree preferred, Associates required.</li><li>
One year employment experience in the Human Services field</li><li>
An equivalent combination of training and experience may be substituted for the educational and employment requirements.</li><li>
Experience and/or training in the care coordination for those with severe mental illness and substance use disorders preferred</li><li>
Basic computer skills required.</li><li>
Must have a valid NYS driver&rsquo;s license.</li></ul><div></div><div><b><br>
