</p><h2 class="jobSectionHeader"><b>Mission</b></h2><p>
The mission of Helpline Youth Counseling (HYC) is to help build individual, family, and community health, well-being, prosperity, and equity. HYC serves clients and community members with trauma-informed, strength-based, person-centered prevention, early intervention, education, and treatment services.</p><h2 class="jobSectionHeader"><b>
DUTIES AND RESPONSIBILITIES:</b></h2><p>
Under the direct supervision of the EMS Program Manager, within the Emergency Medical Service (EMS) program, the Case Manager will:</p><p>
</p><ul><li>Provide individualized case management to clients participating in the EMS program, including developing a Care Plan and monitoring progress toward established plan goals.</li><li>
The EMS Case Manager will provide developmentally appropriate activities and services for program participants.</li><li>
Duties include but are not limited to case management and program services for young adults ages 18-26 including completion of a comprehensive, culturally responsive, strengths-based intake assessment, development of a case management plan, bi-weekly case management meetings, group and 1:1 counseling, life coaching, mentorship from professionals working in relevant fields, physical fitness, job readiness, and job placement.</li><li>
Be responsible for the coordination, planning, implementation, and execution of wraparound program services including in-house HYC and community-based services focused on healing, connecting, and coaching throughout the 5-month program.</li><li>
Maintain case file and provide accurate and timely documentation within agency documentation policies.</li><li>
Complete all client-related program reports within a timely manner.</li><li>
Demonstrate strong oral and written communications skills, including a sound foundation in English grammar, spelling, and punctuation</li><li>
Adhere to and promote HYC&rsquo;s mission and values. Acts as a role model to others exhibiting a high degree of professionalism and ability to work with diverse populations and respect cultural diversity.</li><li>
Capacity to be discreet and a demeanor to maintain confidentiality relating to client and personnel information</li><li>
Adaptive and multi-tasking ability. The individual must easily transition from one task to the next.</li></ul></div><ul><li>
Ability to interact with employees of other departments in a professional and respectful manner.</li><li>
Great work ethic demonstrated by reliability, punctuality, professionalism, diligence, and strong follow-up skills.</li><li>
Must attend all necessary training courses and meetings.</li><li>
Other related duties as assigned.</li></ul><p>
</p><h2 class="jobSectionHeader"><b>REQUIREMENTS:</b></h2><ul><li>
Bachelor&rsquo;s degree in a human services field or equivalent combination of education and relevant work experience.</li><li>
Superb planning and organizational skills with a proven track record for strategic planning and execution and attention to detail.</li><li>
Proficient in Microsoft program applications.</li><li>
Ability to succeed independently within a fast-paced, dynamic, and sometimes unpredictable work environment.</li><li>
Competency to work with the various ethnic/cultural groups in the target area/community.</li><li>
Ability to work a flexible schedule, including evenings and weekends.</li><li>
Great work ethic demonstrated by reliability, punctuality, professionalism, diligence, and strong follow-up skills.</li><li>
U.S. Citizenship or permanent resident status.</li><li>
A valid California driver&rsquo;s license, current auto insurance, a good driving record, and must be cleared to drive for business purposes by our insurance broker.</li><li>
Ability to successfully pass a TB screening test and Live Scan fingerprinting.</li><li>
Ability to pass background check and criminal clearance.</li></ul><p>
</p><h2 class="jobSectionHeader"><b>PREFERRED QUALIFICATIONS:</b></h2><ul><li>
Bilingual in English and Spanish is highly desired.</li><li>
Lived experience with trauma and/or system involvement that can utilize their own life experiences to support, engage, and motivate youth towards the identified goals can be acceptable.</li></ul><p>
