Salary: $25-$35/hr.
Responsibilities of Social Worker:<ul><li>
Focus on Long Term Care, Care Planning and Family Meetings</li><li>
Evaluate patients (existing, new and prospective) based on their needs and limitations</li><li>
Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager</li><li>
Build rapport with patients and their families</li><li>
Coordinate care with PT departments</li><li>
Develop care plans for patients</li></ul> 
Qualifications of Social Worker:<ul><li>
Bachelor&rsquo;s degree required. Master&rsquo;s degree is strongly preferred.</li><li>
Minimum 1+ years' experience as a social worker in a healthcare-related setting. Experience within a nursing home or rehabilitation center is strongly preferred.</li><li>
Knowledge of the rules, regulations, and guidelines that govern nursing care facilities.</li><li>
Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public.</li></ul>
