Position OverviewWe are seeking an APP who is looking for something different than the traditional hospital or corporate healthcare environment.This role offers a high degree of clinical autonomy, flexibility, and independence. As a Wound Care provider with HWMS, you will manage your own patient panel, structure your daily schedule, and provide care in a way that prioritizes outcomes.This is an ideal role for a provider who is confident in their clinical decision-making, values patient relationships, and prefers a more flexible and self-directed work environment.What Makes This Role DifferentUnlike large health systems or corporate medical groups, HWMS offers:<ul><li>Autonomy in Clinical Practice - You will evaluate, diagnose, and treat patients independently, with support from experienced wound care leadership when needed.</li><li>Flexible Scheduling - Your day is not dictated by rigid clinic templates. You will organize your schedule based on patient needs, geography, and clinical priorities.</li><li>Lower Administrative Burden - We minimize unnecessary bureaucracy so you can focus on patient care rather than navigating layers of approval.</li><li>Patient-Centered Care Model - You will have the time and flexibility to truly manage complex wounds and build meaningful relationships with patients.</li><li>Mobile Care Delivery - Care is provided in the home and facility setting, eliminating many of the barriers patients face when accessing traditional wound clinics.</li></ul>Care Coordination<ul><li>Collaborate with home health agencies, skilled nursing facilities, and referring providers</li><li>Communicate treatment plans and updates to the broader care team</li><li>Support transitions of care following hospital or SNF discharge</li><li>Educate patients and caregivers on wound management and prevention</li></ul>Documentation &amp; Compliance<ul><li>Maintain accurate, timely, and compliant documentation</li><li>Ensure documentation meets Medicare and payer requirements</li><li>Follow HWMS clinical protocols and workflows</li><li>Participate in periodic chart reviews and quality initiatives</li></ul>QualificationsRequired<ul><li>Active, unrestricted North Carolina license</li><li>Board Certification (FNP, AGNP, PA-C, or similar)</li><li>DEA license (or ability to obtain)</li><li>Valid driver&rsquo;s license and reliable transportation</li></ul>Preferred<ul><li>Wound care experience or certification (CWOCN, CWON, WCC, or similar)</li><li>Experience in home health, SNF, or post-acute care settings</li><li>Familiarity with Medicare documentation and billing practices</li></ul>Compensation &amp; Benefits<ul><li>Base salary: $110,000 &ndash; $140,000, based on experience</li><li>Bonus potential tied to performance and growth</li><li>Mileage reimbursement</li><li>Malpractice coverage</li><li>Continuing education support</li><li>Flexible scheduling</li></ul>Pay: $110,000.00 - $140,000.00 per yearBenefits:<ul><li>401(k)</li><li>401(k) matching</li><li>Dental insurance</li><li>Flexible schedule</li><li>Flexible spending account</li><li>Health insurance</li><li>Health savings account</li><li>Paid time off</li><li>Retirement plan</li><li>Tuition reimbursement</li><li>Vision insurance</li></ul>Work Location: Remote
