Position Summary: The Patient Care Representative (Clinical &amp; Facilities Support) plays a dual role in ensuring exceptional care for patients seen at our office locations. This position serves as a key communication bridge between office staff, and patients or caregivers. The role involves handling clinical coordination by phone and supporting providers with administrative and care-related workflows. A strong foundation in clinical knowledge, effective communication, and critical thinking is essential. The ideal candidate will thrive in a team-based environment, be detail-oriented, and demonstrate empathy and professionalism when engaging with patients and staff. This position will be based at our Gibsonia office.Expected Duties and Responsibilities:Clinical Phone Support &amp; Patient Triage<ul><li>Serve as the first point of contact for clinical requests via phone, with a primary focus on supporting medication and test prior authorizations.</li><li>Accurately document patient inquiries using EMR dot text protocols and ensure timely, appropriate routing to providers or staff.</li><li>Facilitate communication between patients and providers regarding medication refills, post-discharge needs, test results, and related clinical concerns.</li><li>When necessary, schedule appointments related to medication refills, post-discharge follow-ups, or other clinically driven needs.</li><li>Utilize prior authorization platforms such as CoverMyMeds, Availity, Parachute, and internal logs to track and resolve outstanding requests.</li><li>Apply working knowledge of payer requirements, medication protocols, and office workflows to support timely patient care.</li><li>Collaborate with internal teams to ensure seamless coordination across departments supporting patient care.</li></ul>QualificationsRequired:<ul><li>High school diploma or equivalent</li><li>Minimum of 5 years of clinical experience in a healthcare setting</li><li>Proficiency in EMR systems and internal communication platforms</li><li>Demonstrated understanding of patient care coordination and clinical workflows</li></ul>Preferred:<ul><li>Medical Assistant Certification (CCMA)</li><li>Experience working in post-acute or facility-based care coordination</li><li>Customer service experience in a healthcare setting</li><li>Prior experience supporting providers in a remote or hybrid capacity</li></ul>Job Type: Full-timePay: $18.00 - $21.00 per hourBenefits:<ul><li>401(k)</li><li>Health insurance</li><li>Health savings account</li><li>Paid time off</li><li>Vision insurance</li></ul>Experience:<ul><li>Clinical documentation: 1 year (Preferred)</li></ul>Ability to Commute:<ul><li>Gibsonia, PA 15044 (Required)</li></ul>Work Location: In person
