<div>Primary Duties and Responsibilities
<ul><li>Under the direction of a registered nurse or physician and under assigned administrative supervision, provides care within the scope of the Vocational Nurse Practice Act and regulatory standards. Assists in the assessment, planning, implementation and evaluation of patient care. Follows patient identification procedures, provides appropriate information for all patient procedures, and collects and handles specimens. Assesses and implements prescribed interventions for the patient.
</li><li>Implements and documents discharge plan for each patient. Recognizes that ethical/legal/medical dilemmas exist in the nursing practice and confers with the appropriate resource personnel when making ethical nursing decisions. Participates in maintaining a safe patient care environment. Assist with the care and maintenance of department equipment. Complete and accurate charting of patient records. Complete and accurate record keeping of equipment and quality measures. Collaborate with other disciplines of the health care team to ensure the delivery of care across the continuum, providing timely, safe and quality care. Responsible for providing a safe, HIPAA compliant environment for the patient and the staff. Performs other duties as assigned. Must perform phlebotomy if applicable to department. 
</li><li>Specialty Clinics Only: Performs specialty procedures that include catheters, injections, mohs surgical patients, and injection preparation. Will also coordinate home meds, specialty pharmacy, DME and home care. Will be responsible for coordination of complex care.</li></ul></div>

 <div>Education and Experience
<ul><li>High School diploma or equivalent.
</li><li>2+ years&rsquo; experience as an LVN
</li><li>Basic keyboarding skills and knowledge of Microsoft O&#64259;ce Suite, including Outlook, Word and Excel
</li><li>Basic knowledge of medical terminology
</li><li>Experience with Electronic Medical Records (EPIC) preferred
</li><li>Specialty Clinics: New grad with previous patient care experience (6+ months) will be considered in lieu of 2+ years of LVN experience
</li></ul>License Required
Current LVN License to practice in the State of California 
Certifications Required
Current BLS Certification
Certifications Preferred

LVN II : Neuroscience Specialty Clinic

<div>Primary Duties and Responsibilities
The Clinical Nurse II role provides direct patient care in accordance with the Nurse Practice Act to assigned patients on Medical units. The nurse collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care. This includes providing physical assessments, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, and teaching and discharge needs of the patients assigned. Utilizes the nursing process based on evidence-based best practices to meet patient care needs. Evaluates and adjusts the nursing care process as necessary with consultation as needed, and confers with the appropriate resource personnel when making ethical/legal/medical dilemma decisions. Performs other duties as assigned.
Additionally, the Clinical Nurse II meets the requirements outlined in the Clinical Ladder program respective to his/her department. The Clinical Nurse II demonstrates a commitment to hospital involvement and professional growth by participating in committees, attending related meetings, and presenting education projects and/or supplemental activities.</div>

 <div>Education and Experience
<ul><li>Clinical Nurse II&rsquo;s at Magnet designated facilities only require a BSN or that a BSN program is started within one year of hire date and BSN degree is attained within two years of starting the BSN program.
</li><li>BSN preferred.
</li><li>One year experience in an acute care hospital required.
</li><li>Additional requirements for specific departments are as follows:
<ul><li>HHNB Interventional Radiology, CT Scan, Magnetic Resonance Imaging, Ultrasound, Main X-Ray: Interventional Radiology experience preferred. HHI Main X-Ray: Interventional Radiology and/or vascular access experience preferred. PICC Services: Ultrasound guided vascular access experience is preferred.
</li></ul></li></ul>License Required
Registered Nurse in the State of California - Current and in good standing.
Certifications Required
<ul><li>BLS certification prior to clinical rotation.
</li><li>Department specific certification requirements are as follows:
<ul><li>HHNB Interventional Radiology, CT Scan, Magnetic Resonance Imaging, Ultrasound, Main X-Ray: ACLS certification is required. PALS certification is required within 6 months of hire/transfer to department. NIHSS certification is required within 30 days of hire/transfer to department. Completes annual stroke education per organization requirements.
</li><li>PICC Services: ACLS certification is required.
</li></ul></li></ul>Certifications Preferred
<ul><li>HHNB Interventional Radiology, CT Scan, Magnetic Resonance Imaging, Ultrasound, Main X-Ray: CRN (Certified Radiology Nurse) is preferred.</li></ul></div>
