Home Base Inc. is seeking an ambitious, driven and enthusiastic applicant for the role of Case Manager/Wraparound Facilitator in our Behavioral Health Program! We offer a competitive salary option and benefits package for eligible full-time employees. A full-time Case Manager/Wraparound Facilitator could expect a starting yearly salary of around $47,500.<br>

<br>
Case Manager/Wraparound Facilitator will possess the following qualifications:</div>
<ul><li>Bachelor&rsquo;s Degree in a human service field required.</li>
<li>2-4 years of experience in Case Management and/or Outpatient/ Inpatient behavioral health setting preferred.</li>
<li>1-2 years of experience in leading teams preferred.</li>
<li>Must be able to communicate and demonstrate experience.</li>
<li>Computer skills are essential; must be comfortable with Outlook, Word, Excel, and PDF.</li>
<li>Must have a valid driver&rsquo;s license.</li>
<li>Must be able to pass a NCIC/CIB background check and an APS/CPS Background Check</li>
<li>Must be able to provide three professional references.</li>
<li>Experience with virtual platforms.</li>
<li>Professionalism and customer service skills are essential.</li>
<li>Written communication skills are essential.</li>
<li>Must be able to work independently and without supervision at times.</li>
<li>Must be able to work evenings and weekends</li>
<li>Time management skills are a must</li>
<li>Must have your own reliable transportation</li>
</ul><div>Responsibilities for the Case Manager/Wrap Facilitator include but not limited to:</div>
<ul><li>Provide wraparound services to families with a limit of up to 15 members/families.</li>
<li>Coordinate a comprehensive set of supports, resources, and strategies for each member and family.</li>
<li>Utilizing a strengths-based approach, plan, coordinate, collaborate and facilitate team meetings with the youth, family, and team members.</li>
<li>Ensure services and clinical treatment modalities augment each other for optimal outcomes.</li>
<li>Work with the MCO care manager and family to identify natural supports, and other community resources to meet the family&rsquo;s needs.</li>
<li>Lead the planning team through engagement and team preparation, initial plan development, plan implementation and implementation of a transition plan.</li>
<li>Administer the Child and Adolescent Needs and Strengths assessment tool.</li>
<li>Monitor the implementation of the plan, making sure the family is receiving services identified in the plan.</li>
<li>Initiate and coordinates discharge and after-care planning.</li>
<li>Provide linkage and referral to services and supports as identified in the plan.</li>
<li>Conduct home visits with youth and / or family in accordance with the needs determined in the individualized plan.</li>
<li>Identify and develop formal and informal services / resources available to youth, family, and community.</li>
<li>Identify service gaps and collaborate with community stakeholders and existing services providers for resolution.</li>
<li>Facilitate the development and implementation of an individualized transition plan for members who will reach the waiver&rsquo;s maximum age limit.</li>
<li>Ensure documentation is thorough, accurate and timely.</li>
<li>Ensure that pertinent data is collected and compiled as required.</li>
<li>Participate in weekly/monthly supervision and coaching sessions.</li>
<li>Maintain client records in compliance with organizational goals and objective as well as regulatory guidelines.</li></ul><br>
<div></div><div><b>Physical Requirements:</b></div>
<ul><li>Must be able to walk up and down stairs</li>
<li>Must be able to operate a motor vehicle and follow all motor vehicle laws that apply to West Virginia and the state of which you are a resident</li>
<li>Must be able to lift 5-50 pounds</li>
<li>Must be able to kneel, bend, squat, and stand</li></ul><br>
