Learn more at www.hopephl.org.SummaryThe Case Manager, Supportive Housing (40th Street) is responsible for promoting housing stability, economic mobility, and overall well-being for residents living in a mixed housing model that includes both Permanent Supportive Housing (PSH) and affordable housing units. This role provides housing-focused case management, service coordination, and community-based support to approximately 40 households. Service intensity varies by resident need and program requirements, with higher-intensity services provided to PSH residents and light-touch engagement provided to affordable housing residents.Position Type and Work ScheduleThis is a full-time, exempt position, regularly scheduled to work 40 hours per week. Work schedules are typically Monday through Friday with flexibility to meet resident needs, including occasional evening hours for programming or engagement activities.ESSENTIAL DUTIES &amp; RESPONSIBILITIESHousing Stability &amp; Case Management&middot; Provide housing-focused case management to support long-term housing stability for residents.&middot; Develop, implement, and regularly update individualized service plans.&middot; Maintain consistent contact with residents based on program expectations and acuity.&middot; Support lease compliance, eviction prevention, and resolution of tenancy-related concerns.&middot; Coordinate with property management to address resident needs and housing issues.&middot; Assist residents in maintaining income, benefits, and housing-related resources.Service Coordination &amp; Resource Linkage&middot; Conduct assessments to identify resident needs and goals.&middot; Connect residents to community-based services, including employment, education, behavioral health, healthcare, and financial resources.&middot; Collaborate with internal staff (e.g., Career Counselor, Life Skills, Behavioral Health) and external partners to ensure coordinated service delivery.&middot; Monitor service engagement and address barriers to access.Community Engagement &amp; Programming&middot; Plan and facilitate workshops, groups, and activities that promote independent living skills, financial stability, and community connection.&middot; Encourage resident participation in services and activities.&middot; Support the development of a positive and engaged residential community.Documentation &amp; Compliance&middot; Complete all required documentation in a timely and accurate manner, including HMIS and internal systems.&middot; Maintain audit-ready files in compliance with HUD, OHS, and agency standards.&middot; Track outcomes and support reporting requirements.&middot; Participate in audits, monitoring, and quality improvement activities.Collaboration &amp; Communication&middot; Maintain regular communication with property management, internal teams, and external service providers.&middot; Participate in team meetings, supervision, and case conferencing.&middot; Ensure continuity of care through clear and timely communication.Crisis Response &amp; Safety&middot; Respond to resident crises using trauma-responsive and de-escalation practices.&middot; Follow all agency protocols related to safety, incident reporting, and mandated reporting.Professional Standards&middot; Maintain professional boundaries and confidentiality at all times.&middot; Adhere to HopePHL trauma-responsive care principles, ethical standards, and agency policies.&middot; Participate in required training, supervision, and professional development.Attendance &amp; Reliability&middot; Reliable attendance is an essential function of this position. Conduct comprehensive intake assessments on families who have accepted FES. Deliver case management services to a caseload of families. Follow FES program standards.Essential Duties and Responsibilities:&middot; Adhere to program standards, and the agency&rsquo;s policies and procedures.&middot; Maintain a caseload of up to 15 pending and open cases.&middot; Collaborate with supervisor on referrals daily.&middot; Complete all required documentation by standard dates&middot; Prepare and maintain all client files per the requirements of the FES Table of Contents.&middot; Contact referred families within 3 business days of the referral; make two home visits within 5 business days.&middot; Reach out to DHS contacts via email and phone on all referrals within 6 business days of the receipt of the referral.&middot; Utilize the information from the initial intake questionnaire and the Family Advocacy Support Tool (FAST) to create the Family Service Plan.&middot; Complete at least one visit every 15 calendar days for open cases per family; (all children within a client family living in the home must be seen at least once a month).&middot; Case managers must complete 4 hours of case related work per service month. At least 3 hours of face-to-face contact with the family each month (1 hour must be in the home) and 1 hour of case related service&middot; Enter and update required data on the Community Based Prevention Services (CBPS) database&middot; Case managers are required to complete a school visit within 15 business days from date of intake on all cases with school aged children.&middot; If truancy is identified as a concern, case managers are required to include attendance monitoring as a service plan goal, and complete monthly school visits.&middot; If medical is identified as a concern, case managers are required to include medical monitoring as a service plan goal.&middot; Implement the Family Service Plan with the client by locating and linking them to resources, coordinating services and monitoring the family&rsquo;s use of current and new resources.&middot; Acknowledge the Family Service Plan as a fluid document that must be updated and edited.&middot; Complete all closing documents and protocol by 90th day of an open case and 20th day of a pending case.&middot; Attend bi-weekly supervision on time and prepared with updated and accurate case files.&middot; Attend all required program, unit, and agency events and meetings.&middot; Adhere to agency attendance policy.&middot; Share weekly schedule/calendar with Supervisor and communicate in a timely manner regarding any changes.&middot; Attend agency and department events, and assist with the planning process&middot; Any concerns regarding the safety of clients must be reported to the State Childline and agency and program protocols must be followed.&middot; Complete 20 hours of continuing education each contract year.&middot; Adhere to professional and agency Code of Ethics.Competencies:&middot; Trauma informed skills&middot; Relationship building and engagement skills&middot; Interpersonal understanding&middot; Assessment, planning, and implementation skills&middot; Conflict management and de-escalation skills&middot; Clear and detailed oral and written communication skills&middot; Decisiveness and directness&middot; Organizing and planning ability&middot; Flexible to change&middot; Team skills and collaborative approachRequired Education and Experience:Bachelors degree in social work or another human service field from an accredited institution and at least two years post degree experience in family and children&rsquo;s services.Demonstrates skill in making assessments, conducting intakes, creating comprehensive case plans, and working with families in their own homes.
