 Summary
 
 
 
 Under the direction of the ICMS/PSH Program Manager, the Permanent Supportive Housing (PSH) Case Manager provides intensive case management and supportive services to individuals experiencing homelessness who have been placed in permanent supportive housing. Services include crisis intervention, harm reduction, housing stabilization, employment support, and linkage to community resources. The Case Manager works closely with Property Management, community partners, and other service providers to support clients in maintaining permanent housing and achieving long-term self-sufficiency.
 
 
 
 Essential Functions
 
 
 
 <ul>
 <li>
 Coordinate referrals, intake appointments, interviews, and housing lease-up activities with Property Management and DHS
 </li>
 <li>
 Complete client intakes and facilitate housing placement within established program timelines
 </li>
 <li>
 Develop individualized housing stability plans and connect clients to appropriate supportive services and community resources
 </li>
 <li>
 Provide case management, crisis intervention, housing retention support, and ongoing client engagement
 </li>
 <li>
 Assist clients with employment readiness, training opportunities, and job placement resources
 </li>
 <li>
 Maintain accurate and timely client documentation in CHAMP and client files, ensuring all contacts and required documents are entered within 24 hours
 </li>
 <li>
 Conduct regular follow-up with participants to support housing stability and program compliance
 </li>
 <li>
 Coordinate life skills, enrichment, and supportive service activities that promote self-sufficiency
 </li>
 <li>
 Collaborate with Property Management, DHS, community partners, and internal teams to ensure coordinated care
 </li>
 <li>
 Attend case conferences, team meetings, and community events as required
 </li>
 <li>
 Maintain compliance with HIPAA, 42 CFR Part 2, and all funding and program requirements
 </li>
 <li>
 Represent the agency professionally and maintain appropriate professional boundaries
 </li>
 <li>
 Perform other duties as assigned

 Minimum Qualifications - Knowledge, Skills And Abilities Required
 
 
 
 <ul>
 <li>
 Associate&rsquo;s Degree in Social Services, Business, Housing, or a related field from an accredited institution; OR two (2) years of experience in social services, case management, homelessness services, or a related field
 </li>
 <li>
 Knowledge of case management practices, crisis intervention, housing services, and community resources
 </li>
 <li>
 Proficiency with Microsoft Office Suite and database systems
 </li>
 <li>
 Strong written and verbal communication skills
 </li>
 <li>
 Ability to work effectively with individuals from diverse cultural, ethnic, and socioeconomic backgrounds
 </li>
 <li>
 Must successfully complete employment eligibility verification and background clearance requirements
 </li>
 <li>
 TB clearance required prior to hire or within seven (7) days of employment and renewed annually
 </li>
 <li>
 CPR and First Aid certification required within 30 days of employment
 </li>
 <li>
 Valid California Driver&rsquo;s License, reliable transportation, and current auto insurance required
 </li>
 <li>
 Bilingual candidates are encouraged to apply

 Experience with HMIS and/or CHAMP preferred

 Environmental Conditions (Working Conditions)
 
 
 This position operates within residential and community-based settings and requires local travel. The incumbent may work in environments with multiple residents, staff, and community partners present and must be able to adapt to changing priorities and a fast-paced work environment.
 
 
 Physical Requirements
 
 
 The incumbent typically spends a considerable amount of time sitting, standing, walking, driving, typing, carrying materials up to 25 pounds, speaking, listening, and interacting with clients, residents, and community partners.
 
 
 Mental Requirements
 
 
 This position requires the ability to handle frequent interruptions, changing priorities, and multiple deadlines while maintaining professionalism and quality service delivery.
