</p><p><b>Job Summary:</b> The THRIVE Case Manager would provide a holistic approach to case management, aiding in facilitation of employment, career plans, systemic referral procedures and housing stability. Serving as a &ldquo;Hub&rdquo; for the THRIVE collective, ensuring that participants do not fall through the cracks of the referral system and providing accountability to the participant. This position will be responsible for maintaining an active case load of participants, data entry, as well as, preparation and submission of reports.
</p><div><b>Essential Functions</b><br>
<ul><li>Participate in the development and implementation of case finding and participant recruitment initiatives.
</li><li>Perform screening, intake, stabilization plans and orientation functions with participants in the Winchester, Frederick, and Clarke County area to facilitate participant entrance into appropriate programs and services.
</li><li>Act as case manager for participants, providing monthly case management services including information and referrals to services needed through the THRIVE program as well as additional community resources.</li><li>Implement training and interventions to further career development of participants, including Get Onboard!, job seeking support, employment retention support, and barrier resolution.
</li><li>Act as liaison between participants, employers, landlords/property managers, referral and funding sources.
</li><li>Provide post-employment support and assist participants to identify and utilize natural support systems.
</li><li>Educate participants and community organizations concerning the THRIVE program and services.
</li><li>Act as an advocate and provide accountability for participants.
</li><li>Cooperate and maintain good rapport with participants, employers, landlord/property managers, THRIVE partners and referring agencies.
</li><li>Maintain all relevant participant records, case notes and other documentation including attendance records and progress reports.
</li><li>Maintain data entry and outcome tracking. Complete and submit semi-annual reports, other reports, and program proposals to funding source(s) as directed.
</li><li>Develop and practice participant retention monitoring to reduce disengagement and promote long term stabilization.</li><li>Complete progress reports within the THRIVE program and submit fee-for-service invoices as directed.
</li><li>Assist with program evaluation and community needs assessment to identify program and service needs.
</li><li>Participate in development and submission of program and service proposals to expand and diversify program funding as directed.
</li><li>Participate in agency meetings and committees as directed.<br>

<b>Education and Experience</b><br>
<ul><li>Associate's degree or high school diploma and two years of relevant experience required, Bachelor's degree preferred.
</li><li>Personal experience with the public assistance system preferred.
</li><li>Must possess a clean driving record and provide own transportation.
