The Primary Care/Family Practice Physician Associate (Assistant) or Nurse Practitioner is responsible for the medical services. Horizon Health and Wellness is an Integrated Health Care company. Horizon Health and Wellness provides a multi-directional integrated behavioral health, primary care and wellness services designed to improve individual health outcomes and reduce healthcare costs. This position will actively engage in the provision of culturally responsive, integrate services as well as support and/or participate in initiatives designed to foster and promote integrated service delivery.
Qualifications:<ul><li>
Licensed in the State of Arizona as PA or NP and National Certification if applicable.</li><li>
Possess current DEA registration.</li><li>
Have no pending actions by board of medicine in Arizona or any other state.</li><li>
Have current unencumbered license to practice as a physician assistant and to prescribe medications in the State of Arizona.</li><li>
Possess valid drivers license with acceptable driving record, telephone and reliable transportation.</li><li>
Willingness to participate in trainings and continuing education.</li><li>
Ability to analyze and assess situations and patients.</li><li>
Ability to make sound judgments relating to patient care.</li><li>
Ability to complete accurate patient records documentation.</li><li>
Certification in first aid and CPR.</li><li>
Proof of good health by initial Physical examination and negative TB test.</li><li>
Must have strong oral and written communication skills.</li><li>
Demonstrated knowledge of referral resources available to participants.</li><li>
Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.</li><li>
Must obtain and maintain a fingerprint clearance card and successful completion of background check.</li><li>
Must be at least 21 years of age or older.</li></ul></div>

As a SMI Clinical Coordinator you will oversee our outpatient behavioral health services team and ensure that quality services are provided to our clients. You will participate in hiring, training, mentoring, and supervising staff, as well as serving as a clinical resource for consultation for staff.
Qualifications<ul><li>
High School Diploma/GED with 4 years&rsquo; experience providing client care in a behavioral health setting.
</li><li>2 years&rsquo; experience providing behavioral health case management services.</li><li>
Must be at least 21 years of age or older.</li><li>
Must be able to obtain and maintain a fingerprint clearance card and successfully complete background check.</li></ul>
Qualifications Preferred<ul><li>
Bachelor&rsquo;s degree in a behavioral health related field.</li><li>
Bilingual English/Spanish
</li><li>Demonstrates knowledge of special populations such as substance abuse.</li><li>
