Position Summary:

HousingPlus is hiring an social worker to join our growing company. The Social Worker is responsible for providing trauma-informed, evidence-based clinical and supportive services to women and families residing in a permanent supportive housing program. Participants include individuals and families impacted by serious mental illness (SMI), substance use disorders (SUD), histories of justice involvement/reentry, physical disabilities, and other complex needs. The Social Worker will work collaboratively with case managers and the broader support team to ensure participants with higher needs receive comprehensive, coordinated care that promotes housing stability, wellness, and self sufficiency.

Trauma-Informed Practice: Ability to recognize and respond to the impact of trauma on individuals and families.

Clinical Assessment &amp; Intervention: Strong skills in psychosocial assessment, treatment planning, and evidence-based interventions.
Collaboration &amp; Teamwork: Works effectively with case managers, property management, and external providers to support participant goals.

Cultural Humility &amp; Responsiveness: Demonstrates sensitivity to the lived experiences of diverse populations, including those impacted by systemic inequities.

Crisis Management: Skilled in de-escalation, safety planning, and responding to mental health or substance use crises.

Ethics &amp; Professional Judgment: Adheres to NASW Code of Ethics and maintains professional boundaries and confidentiality.

Communication: Clear verbal and written communication skills with participants, staff, and external stakeholders.

Duties/Responsibilities:
<ul>
<li>Provide individual, family, and group interventions utilizing evidence-based practices such as motivational interviewing, cognitive-behavioral approaches, harm reduction, and trauma-informed care.</li>
<li>Conduct psychosocial assessments and develop individualized service and treatment plans in collaboration with participants and the multidisciplinary team.</li>
<li>Work closely with case managers to identify and support participants with higher acuity needs, providing clinical guidance and direct interventions as appropriate.</li>
<li>Facilitate referrals and connections to external providers for mental health, medical, legal, vocational, and community-based services.</li>
<li>Provide crisis intervention, de-escalation, and safety planning as needed.</li>
<li>Maintain timely and accurate documentation in compliance with agency policies and funder requirements.</li>
<li>Participate in case conferences, staff meetings, and interdisciplinary care coordination.</li>
<li>Support program initiatives aimed at enhancing participant engagement, housing stability, and family well-being.</li>
<li>Provide in home and in office supports. Conduct visits home visits as needed.</li>
<li>Provide clinical supervision or mentorship to social work interns as assigned.</li>
<li>Other tasks as assigned</li>
</ul>
Required Skills/Abilities:
<ul><li>Minimum of 2 years' experience working with populations with SMI, SUD, homelessness, reentry, or related challenges.</li>
<li>Knowledge of permanent supportive housing, harm reduction, and recovery-oriented practices preferred.</li>
<li>Experience working with families and women with complex needs is strongly desired.</li>
<li>Proficiency with electronic health record or case management systems.</li>
<li>Must be able to walk distances of .25 miles, walk over uneven ground, and climb a minimum of 3 flights of stairs.</li>
<li>Bilingual skills a plus.</li>
</ul>
Education and Experience:
<ul>
<li>Master's Degree in Social Work (MSW) required.</li>
<li>New York State LMSW or LCSW licensure strongly preferred.</li>
</ul>
Additional Schedule Details:
<ul>
<li>Based in a congregate or scattered site permanent supportive housing program setting.</li>
<li>Combination of office-based work, participant home visits, and community outreach.</li>
<li>May require evening or weekend hours depending on participant needs.</li>
