Position Summary: 
We are seeking a dynamic and clinically strong Hospital Liaison to serve as the primary connection between our skilled nursing facilities and hospital partners. This role is ideal for a Respiratory Therapist (RT) with experience in skilled nursing settings who can effectively evaluate patients, build relationships, and facilitate appropriate admissions&mdash;particularly for higher-acuity and ventilator-dependent patients. 


Key Responsibilities:
<ul>
<li>Serve as the primary liaison between hospitals, case managers, and our skilled nursing facilities
</li>
<li>Conduct on-site or remote clinical evaluations of potential admissions, with a focus on respiratory and high-acuity patients
</li>
<li>Collaborate with hospital discharge planners, physicians, and care teams to coordinate safe and efficient transitions of care
</li>
<li>Identify appropriate placement opportunities within our facilities based on clinical needs and capabilities
</li>
<li>Educate hospital partners on our services, including ventilator care and respiratory programs
</li>
<li>Build and maintain strong referral relationships to support census growth and quality admissions
</li>
<li>Communicate closely with internal clinical and admissions teams to ensure seamless patient transitions
</li>
<li>Participate in care planning discussions and provide clinical insight when needed
</li>
<li>Track referral activity and follow up on pending admissions</li></ul> 

Qualifications:
<ul>
<li>Licensed Respiratory Therapist (RT) required
</li>
<li>Minimum of 2+ years of experience in a skilled nursing facility (SNF) setting
</li>
<li>Experience with ventilator management and high-acuity patients strongly preferred
</li>
<li>Strong clinical assessment and critical thinking skills
</li>
<li>Excellent communication and relationship-building abilities
</li>
<li>Ability to work independently while managing multiple priorities
</li>
<li>Familiarity with hospital discharge processes and post-acute care coordination
</li>
<li>Valid driver&rsquo;s license and willingness to travel locally between hospitals and facilities</li></ul>
