The H.U.F. Center is seeking a compassionate and reliable Case Manager to support adults with mental health and co-occurring disorders in a psychosocial day support setting.<br>
As a Case Manager with Hope, Unity, and Freedom (H.U.F.) Center, you play a vital role in supporting adults with primary mental health diagnoses, co-occurring disorders, and/or substance use challenges. This position focuses on providing psychosocial day support services that promote independence, stability, and overall well-being.<br>
This is a hands-on, highly interactive role where you&rsquo;ll work closely with members throughout the day&mdash;providing direct support, facilitating interventions, and ensuring all services are clearly and accurately documented. The role requires consistency, strong interpersonal skills, and the ability to work with a diverse and sometimes high-acuity population.</p><h3 class="jobSectionHeader"><b><br>
Key Responsibilities</b></h3><ul><li>
Provide direct support and case management services to assigned members</li><li>
Observe, engage, and interact with members daily</li><li>
Complete timely documentation within 24 hours (daily notes, monthly/quarterly reports, ISPs)</li><li>
Facilitate group and individual psychosocial interventions as needed</li><li>
Assist with daily program operations (meals, breaks, general support)</li><li>
Collaborate with team members, caregivers, and external providers</li><li>
Support members with ADLs/IADLs as appropriate</li><li>
Maintain a safe, structured, and supportive environment</li></ul><h3 class="jobSectionHeader"><b>

Qualifications</b></h3><ul><li>
Must have Virginia QMHP</li><li>
Bachelor&rsquo;s degree in Human Services, Social Work, Psychology, or related field preferred</li><li>
Experience in mental health, behavioral health, or substance use preferred</li><li>
Strong communication and organizational skills</li><li>
Ability to complete documentation accurately and on time</li><li>

Our work helps to decrease psychiatric hospitalization and provide stabilization of individuals with less intervention in the community. By providing daily routine and structure in a safe environment, each individual is able to achieve their best and improve their quality of life.</p><p><b>
Position Summary</b></p><p>
As a Medication Technician with the Hope, Unity, and Freedom (H.U.F.) Center, you are provided the opportunity to make a difference in the daily functioning of adults in need. Med Techs are responsible for completing progress notes, administering medications to clients under the overview of Program Nurses, and ensuring all safety measures are always being adhered to, among other duties. Med Techs are the primary support system for those in their care while at the H.U.F. Center, improving the lives of those around them daily.</p><p><b>
Position Responsibilities</b></p><ul><li>
Responsible for completing all pertinent clinical documentation.</li><li>
Co-facilitate psychosocial activities and therapeutic integration activities.</li><li>
Responsible for ensuring all safety measures are always adhered to and followed.</li><li>
Monitor clients at all times throughout the premises.</li><li>
Provide direct care services specific to ADLs/IADLs with clients.</li><li>
Participate in weekly team meetings.</li><li>
Transport clients to activities in the community after successfully completing vehicle training.</li><li>
Other duties as assigned by the Program Manager and/or Program Nurse.</li><li>
During the COVID-19 pandemic, it is required that all employees wear pre-approved masks as designated by the CDC guidelines, including surgical masks, KN95 masks, or N95 masks.</li></ul><p><b>

Requirements:
<p>
Role Qualifications:</p><ul><li>
At a minimum, must possess a medication administration certification approved by the Virginia Board of Nursing; at maximum, the applicant is a Certified Nursing Assistant under the Virginia Board of Nursing</li><li>
Must have at least 1 year of documented experience working with mental health needs.</li><li>
Must be able to complete and pass: a CPR/First Aid certification; Handle with Care physical and/or verbal intervention training; annual tuberculosis (TB) screening test; and clear criminal background and central registry check.</li><li>
Must have a valid Virginia Driver&rsquo;s License.</li><li>
Strong and assertive personality.</li><li>
Excellent verbal and written communication; required to complete documentation.</li></ul></div>
