What You&rsquo;ll Be Doing:<ul><li>
Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines</li><li>
Plan nursing interventions for each resident based on individual needs and monitor the implementation</li><li>
Evaluate the effectiveness of care interventions, identify problems and trends and develop alternative interventions.</li><li>
Conduct family and resident education</li><li>
Administer medications and treatments timely and according to center policy</li><li>
Transcribe physician&rsquo;s orders and chart progress notes in an informative, factual manner that reflects the care administered as well as the resident&rsquo;s response</li><li>
Identify and report changes in condition to supervisor, physician and family</li><li>
Accurately identify skin changes and follow skin management protocols</li><li>
Order and report diagnostic tests timely</li><li>
Follow Pharmacy policy and procedures for ordering and delivering medications.</li><li>
Coordinate resident&rsquo;s rehabilitation program with therapists to ensure rehabilitation needs are met</li><li>
Monitors that CNA documentation is completed daily</li><li>
Ensures CNA assignments are completed on time</li><li>
Perform additional nursing care responsibilities as required and based on State Nurse Practice Act.</li></ul>
Requirements:<ul><li>
Active state RN license</li><li>
CPR/BLS Certification</li><li>
Experience in Long Term Care or Skilled Nursing preferred</li><li>
Excellent technical, assessment and documentation skills</li><li>
Strong analytical and problem-solving skills</li><li>
Ability to lift 25 or more pounds</li><li>
Ability to stand, bend, and walk for extended time periods</li><li>
Team player, strong communicator, passionate about senior care</li></ul>
Salary/ Wage Range
Compensation for the role will depend on several factors, including a candidate&rsquo;s qualifications, skills, competencies and experience, and may fall outside of the range shown.
