Job Description 
Metro is looking for a motivated and passionate PER DIEM Physical Therapist Assistant to provide in-home care under the supervision and direction of a licensed Physical Therapist. Responsibilities:<ul><li>
Accept and treat patients within your designated geographic area</li><li>
Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT&rsquo;s plan of care</li><li>
Monitor and report changes in patient status to the supervising PT in a timely manner</li><li>
Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care</li><li>
Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT</li><li>
Assist the PT in updating care plans based on patient progress and response to treatment</li><li>
Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards</li><li>
Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT</li><li>
Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice</li></ul> 

Qualifications 
<ul><li>
Physical Therapist Assistant Program Graduate</li><li>
Licensed Physical Therapist Assistant in the state of New York or eligible.</li><li>
Mandatory Certifications: First Aid, AED, &amp; CPR Certifications.</li><li>
Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.</li><li>
Ability to travel to patients&rsquo; homes within the assigned geographic territory</li></ul></div>
