As an Adult Case Manager, you will play a key role in assisting individuals with intellectual disabilities or autism to access resources to live full and inclusive lives. You will work closely with both the individual and their identified support group, and you will provide thorough assessments of strengths and needs, comprehensive planning of services, advocacy, outreach and resource coordination unique to every person.
