Job Type: Full timeDescription:*This position requires you to have a vehicle with which to transport clients to appointments and other activities in the community. The role of the Integrated Community Supports (ICS)/Individualized Home Supports (IHS) Professional is to provide support and/or training for people with mental health, physical and developmental disabilities. ICS/IHS Professionals help individuals achieve their goals in the areas of community engagement, health and wellness, household management, and adaptive skills. This position entails having a caseload of clients and providing support either at an onsite ICS location in the St Paul area or in their independent housing site in the twin cities.In this position, you will:<ul><li>Review 245D paperwork, including support plans, and utilize as a guide on how to work with your client</li><li>Collaborate with clients' support team members on ways to assist and care for the client</li><li>Determine which goals your client would like to accomplish and develop a plan on how to assist them in achieving these goals</li><li>Drive clients to community outings, stores, and appointments</li><li>Provide supervision and training to clients on how to complete routine household care and maintenance</li><li>Assist clients in researching and accessing community resources</li><li>Assess clients' risks and assist them in ways to mitigate these risks</li><li>Build skills with clients which could include, but are not limited to: teaching clients to cook, meal plan, budget, complete paperwork and manage their appointments</li><li>Guide and assist clients with their activities of daily living</li><li>Implement strategies with clients on ways to manage their behaviors</li></ul>Expectations:<ul><li>Work hours are Monday thru Friday (Full time)</li><li>You will meet weekly with your supervisor to discuss your schedule and consult about client issues.</li><li>You will complete Relias and other IHS trainings within your first two weeks of employment. After that, Relias trainings will need to be completed on an annual basis.</li></ul>Required Qualifications:<ul><li>Must be at least 18 years or older with a High School diploma or GED.</li><li>Proof of Valid Driver's license and car insurance.</li><li>Must pass criminal background check administered by the Department of Human Services (DHS)</li><li>Must provide proof of COVID-19 vaccination(s)</li><li>Can work well with a diverse population, including people with low-income, who struggle with substance abuse, criminal backgrounds, and people with developmental, mental and physical disabilities</li><li>Ability to manage time effectively</li><li>Some knowledge of Microsoft office, including Word and Excel</li></ul>Preferred Qualifications:<ul><li>Bachelors degree in Social Work, Psychology, Sociology, Human Services or related field</li><li>Experience in working with individuals with disabilities</li><li>Knowledge of public programs, medical assistance and Medicaid waivers</li><li>Experience in researching and accessing different community resources</li></ul>Job Type: Full-timePay: $22.00 per hourBenefits:<ul><li>401(k)</li><li>Flexible schedule</li><li>Life insurance</li></ul>Work Location: In person
