Clinic Hours of Operation:
Monday - 8:00 a.m. - 5:00 p.m.
Tuesday - 8:00 a.m. - 6:00 p.m.
Wednesday - 8:00 a.m. - 6:00 p.m.
Thursday - 8:00 a.m. - 6:00 p.m.
Friday - 8:00 a.m. - 4:00 p.m.
Saturday (1st and 3rd) - 8:00 a.m. - 12:00 p.m.

Requirements:
<ul><li>Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited school of medicine</li><li>
Completion of an accredited Residency Program</li><li>
Licensed or eligible for licensure in Indiana</li><li>
Board-Certified or Board-Eligible</li><li>
Active and unrestricted DEA license or ability to obtain prior to start</li><li>
Commitment to the mission of Indiana Health Centers, Inc.</li><li>
Prefer working at an FQHC, CHC, or PCMH</li></ul>
