Job SummaryThe Medical Records &amp; Documentation Executive (Fresher) will assist in maintaining accurate and confidential patient medical records. The role involves basic documentation, data entry, coordination with clinical staff, and learning healthcare record-keeping processes under supervision.Key ResponsibilitiesMedical Records Support<ul><li>Assist in creating and maintaining patient files (physical and digital)</li><li>Enter patient details, treatment notes, and reports into the system</li><li>File and retrieve medical records as required</li><li>Ensure documents are complete and properly organized</li></ul>Documentation &amp; Accuracy<ul><li>Check basic completeness of patient forms and reports</li><li>Follow documentation formats and clinic procedures</li><li>Report missing or incorrect documents to seniors</li></ul>EMR / Data Entry<ul><li>Perform data entry in EMR / hospital software</li><li>Update records after consultations and procedures</li><li>Maintain daily logs and registers</li></ul>Coordination &amp; Support<ul><li>Coordinate with doctors, nurses, and front office staff for documentation</li><li>Assist billing team with required medical documents</li><li>Respond to internal requests for patient records</li></ul>Confidentiality &amp; Compliance<ul><li>Maintain strict confidentiality of patient information</li><li>Follow clinic policies, data protection, and ethical standards</li></ul>Eligibility &amp; Skills<ul><li>BSC in Botany or Zoology</li><li>Freshers welcome</li><li>Basic computer knowledge (MS Word, Excel)</li><li>Willingness to learn EMR / hospital systems</li><li>Attention to detail and good organizational skills</li><li>Good communication and teamwork</li></ul>Training &amp; Growth<ul><li>On-the-job training will be provided</li><li>Opportunity to grow into Medical Records Officer / Executive role</li></ul>Reporting To<ul><li>Medical Records Officer / Clinic Manager</li></ul>Job Type: Full-timePay: ₹12,000.00 - ₹20,000.00 per monthEducation:<ul><li>Bachelor's (Preferred)</li></ul>Location:<ul><li>Thirumala, Thiruvananthapuram, Kerala (Preferred)</li></ul>Work Location: In person
