InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare, social engagement, and exercise to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at an InnovAge center in their community. Each person in the program has a team of healthcare experts and a personal care plan that is focused on providing a personalized experience.</div><div></div><div><br>
As a <b>Psychiatric Nurse Practitioner (PMHNP) </b>you are an integral part of developing and delivering a personal care plan, ensuring high quality, clinically appropriate, team-based behavioral health care. Physical attendance at an InnovAge center(s) is essential to performing the job duties with the option to travel to meet participants outside of center. No on-call or weekend hours required.</div><div></div><div><br>

Confidently and swiftly navigate ambiguous situations to manage participants&rsquo; care in real time</li><li>
Take bold action using evidenced-based medication management to support acute needs</li><li>
Emphasize gradual dose reduction</li><li>
Natural ability to integrate with primary care and other Interdisciplinary Team (IDT) members</li><li>
Adapt interpersonally to meet participant and IDT clinical needs in real time</li><li>
Pioneer InnovAge&rsquo;s developing behavioral health integration program</li><li>
Serve as mentor/educator to PCP&rsquo;s regarding psychotropic medication management and behavioral change</li></ul><div>
Clinical Care &ndash; 70%</div><ul><li>
Provide in-person clinical assessments of participants, prescribe and deprescribe, identify treatment protocols, and on-going medication management in centers and via telehealth</li><li>
Coordinate care with Behavioral Health Providers, PCPs, and IDT</li><li>
Consult with IDT staff on behavioral interventions, nursing practice, and psychopharmacology, including writing orders for medications, as appropriate</li><li>
Provide integrated therapeutic care</li><li>
Monitor psychiatric hospitalization care and follow-up disposition planning</li><li>
Respond to urgent calls from clinics, center staff, and facilities, with a solution-focused approach</li><li>
Monitor response to psychotropic medications</li></ul><div>
Administration &ndash; 30%</div><ul><li>
Educates IDT to manage and resolve behavioral complexities or placements</li><li>
Liaisons with external psychiatric providers to ensure appropriate referrals, continuity of care, monitoring/follow up and report to IDT</li><li>
Provide in-service education at PACE centers; participate in staff meetings as needed</li><li>
Contribute to case review, consultation, and utilization review</li><li>
Research and utilize evolving psychiatric treatments, PACE service requirements, and legislation</li><li>
Participates in IDT meetings, conferences, committees, and events as requested</li></ul><div><b>
REQUIRED</b></div><ul><li>
Current State issued Psychiatric Nurse Practitioner/PMHNP license</li><li>
2+ yrs in psychiatric care and medication management</li><li>
5+ yrs nursing with at least 2yrs as Nurse Practitioner</li><li>
DEA Certification with full prescriptive authority</li><li>
Proficiency with standard Microsoft Office applications and EMR systems</li><li>
Current CPR certification or the ability to obtain CPR certification in first 30 days of employment</li><li>
Requires personal transportation, valid state issued driver&rsquo;s license, good driving record and auto insurance as required by law.</li></ul><div><b>
PREFERRED</b></div><ul><li>
Experience working with geropsychiatric populations</li><li>
Licensure or certification in substance use treatment</li><li>
Training or experience with group and family interventions</li><li>
Experience with conflict management, mediation, de-escalation, and crisis mitigation</li><li>
Knowledge of the chronic disease management and psycho-social implications</li><li>

Psychiatric Nurse Practitioner

</div><div>Under direct supervision, performs activities associated with drug distribution, ordering and control for medication dispensing. Direction and guidance are performed by a manager/supervisor, Pharmacist, or higher-level Pharmacy Technician.</div><div></div><div><br>
Essential Functions and Work Responsibilities</div><div></div><ul><li><br>
Receives electronic prescriptions, weekly batches or refill requests and accurately and efficiently fills the prescriptions under the direction of the Pharmacist. Performs pharmacy tasks such as handling, counting, mixing, and packaging medications.</li><li>Selects and imports orders for automation equipment dispensing, prints labels, and maintains order management to dispense prescriptions. Operates and monitors robotic machines that dispense medications.</li><li>Assists in entering minor edits to record correct dispensing quantity and manufacturer NDC used to dispense prescription order.</li><li>Prepares blister packs.</li><li>Prepares and stages pharmaceutical supplies and drugs in the correct bins for delivery.</li><li>Performs reclamation duties.</li><li>Uses pharmacy software to check and/record data about prescriptions.</li><li>Maintains proper storage and security conditions for the drugs. Cleans and helps to maintain equipment and work areas.</li><li>Receives, stores and logs inventory and returns (upon Pharmacist approval), verifies quantities against invoices, and informs supervisors of stock needs or shortages. Remains aware of expiry dates and rotates stock to avoid dispensing out of date products.</li><li>Acts as a liaison between our customers and their providers and our pharmacy, receives and triages incoming phone calls and messages to answer questions, fill prescription orders or handle other requests to ensure that clients have a positive experience.</li><li>Establishes and maintains patient profiles, including lists of medications taken by individual patients, and uses our advanced proprietary software system to document medication care plans.</li><li>Uses pharmaceutical technician expertise to ensure compliance, efficiency, and quality of operations. Participates in company quality initiatives, recognizing opportunities for improvements and reporting quality assurance issues. Secures information in accordance with the privacy and security guidelines of the Company and applicable regulations such as HIPAA.</li></ul><div></div><div><br>
