Inspire Hospice and Palliative Care

<div><p>Inspire Hospice &amp; Palliative Care is seeking a compassionate and dedicated <b>Physical Therapist (PT)</b> to join our interdisciplinary care team on a <b>Paid Per Visit (PPV)</b> basis. This role provides personalized therapy services to hospice patients in their homes, helping improve comfort, mobility, safety, and quality of life.</p>

Position Summary</b></h3>
<p>
The Physical Therapist plans, organizes, implements, and evaluates physical therapy treatment programs based on physician referrals and patient assessments. This position works collaboratively with the Interdisciplinary Group/Team (IDG/IDT) to provide quality, patient-centered hospice care.</p>
<h3 class="jobSectionHeader"><b>
Responsibilities</b></h3>
<ul><li>Perform clinical assessments to identify patient needs and therapy appropriateness</li>
<li>Develop, document, and revise individualized Plans of Care (POC)</li>
<li>Complete timely clinical documentation, progress notes, and physician orders</li>
<li>Provide therapy treatments for neuromuscular, neurological, and orthopedic conditions</li>
<li>Educate patients and caregivers on transfer techniques and safe equipment use</li>
<li>Utilize therapy equipment appropriate to patient diagnoses and needs</li>
<li>Collaborate with the hospice interdisciplinary team to coordinate care</li>
<li>Maintain infection control standards and safety practices</li>
<li>Accept assignments consistent with education, experience, and competency</li>
<li>Participate in continuing education and maintain licensure requirements</li>
<li>Promote the mission and values of Inspire Hospice &amp; Palliative Care</li>
</ul><h3 class="jobSectionHeader"><b>Qualifications</b></h3>
<h4 class="jobSectionHeader"><b>Education</b></h4>
<ul><li>Graduate of a CAPTE-approved Physical Therapist Education Program</li>
<li>Doctor of Physical Therapy (DPT) required<br>
(<i>Exceptions may apply for education/licensure obtained prior to 2009 or outside the United States</i>)</li>
</ul><h4 class="jobSectionHeader"><b>Licensure</b></h4>
<ul><li>Current state Physical Therapist license or compact PT license where applicable</li>
</ul><h4 class="jobSectionHeader"><b>Experience</b></h4>
<ul><li>Two years of experience in acute care or rehabilitation preferred</li>
</ul><h4 class="jobSectionHeader"><b>Skills &amp; Requirements</b></h4>
<ul><li>Strong therapy and patient assessment skills</li>
<li>Knowledge of durable medical equipment</li>
<li>Excellent communication and interpersonal skills</li>
<li>CPR certification preferred/required per hospice policy</li>
<li>Reliable transportation and valid auto liability insurance required</li>
</ul><h3 class="jobSectionHeader"><b>Work Environment &amp; Physical Requirements</b></h3>
<ul><li>Travel to patient homes and healthcare facilities within assigned territory</li>
<li>Ability to lift, bend, push, pull, and stand for extended periods</li>
<li>Ability to work in emotionally sensitive situations with terminally ill patients and families</li>
<li>Exposure to varying weather conditions and healthcare environments</li>
