Summary: 
Accountable for acting as part of a team in the service delivery plan for persons with mental health and or addiction disorders and the care, guidance, supervision and training of clients in a supported or supervised housing program.
Essential Duties &amp; Responsibilities:
<ul><li>Implements programs and monitors clients within a supported/supervised living program</li><li>
Functions as a member of an interdisciplinary team and participates in service delivery plan process for clients</li><li>
Provides guidance and support for clients to develop independent living skills</li><li>
Participates in and assists clients with activities of daily living</li><li>
Assists clients in accessing goods, services and supports required for independent living; reinforces appropriate client behavior through modification techniques</li><li>
Responsible for clients health and safety</li><li>
Observes and reports client behavior</li><li>
Serves as advocate for government subsidies and re-determination hearings</li><li>
Transports clients Acts as liaison to clients and their families or various community agencies or organizations</li><li>
Provides concise and accurate reports and documentation</li><li>
May make recommendations on policies or standards
</li><li>May administer and track medications with appropriate certification</li><li>
May administer first aid in emergency situations</li><li>
May schedule and attend appointments with clients</li><li>
Performs related duties as required</li></ul>
 *All agency staff are required to attend all mandatory department/agency meetings and trainings*
Schedule:
Tuesday - Saturday, 8:00 am - 4:30 pm
Requirements:

Education &amp;/Or Experience:
High School Diploma plus must be working towards an Associate Degree. Two years&rsquo; experience working with substance abusers in a residential setting preferred. Ability to work with diverse population. A familiarity with Twelve Step Program. Working experience at least one year.
