As a Hospice Registered Nurse Case Manager (RN), you will:<ul><li>
Oversee the implementation and ongoing assessment of the patient's plan of care alongside the spiritual care coordinator, social worker, hospice aide and medical director
</li><li>Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs
</li><li>Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve</li></ul>
To qualify for a Hospice Registered Nurse Case Manager (RN) with us, you will need:<ul><li>
Education: Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required; Bachelor of Science in Nursing (BSN) preferred
</li><li>Licensure: Current license to practice as a Registered Nurse (RN) in the state associated with this position.
</li><li>Current CPR/FA certification
</li><li>Reliable transportation to/from care sites and/or work locations
</li><li>One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting preferred</li></ul>

As an Account Liaison, you will:
Prepare sales plan with Executive Director/Administrator, including forecasts, target accounts, contacts within accounts and budget for achieving objectives
Cause profitable growth by establishing and maintaining contact and relationships with referral sources through which prospective clients might be reached and influenced
Develop and implement marketing plan for Home Health and Hospice Services
Increase awareness of and interest in Interim HealthCare and by preparing and conducting presentations regarding Interim HealthCare and Hospice to organizations where prospective clients might be reached or influenced
To qualify for an Account Liaison with us, you will need:
Education: Bachelor's Degree in a business-related subject or equivalent work experience and training
Five (5) years' successful sales experience with comparable customers, preferably to Hospitals and/or Nursing Home
Knowledgeable in Hospice required
Experience in selling a service
Proven ability to work independently with minimal supervision while being an active team player
