Brief overview of Responsibilities:
<ul>
<li>Support the Registered Nurse in delivering each patient&rsquo;s plan of care</li>
<li>Assist with specialized procedures under the RN&rsquo;s direction and the physician&rsquo;s orders</li>
<li>Contribute to planning, implementing, and regularly evaluating patient care plans</li>
<li>Recognize how social and economic challenges impact patients, and provide emotional and physical support for them and their families</li>
<li>Help educate patients and families on proper self-care techniques</li>
<li>Actively participate in team and staff meetings to promote coordinated care</li>
<li>Document clinical and progress notes after each patient visit</li>
<li>Monitor and report any symptoms, reactions, or changes in patient condition to the appropriate team member</li>
<li>Participate in a rotating weekend on-call schedule</li>
<li>Perform other duties as needed</li></ul> 


Qualifications:
<ul>
<li>Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia</li>
<li>At least one year of home health experience preferred</li>
<li>Graduate of an accredited nursing school.</li>
<li>Must have a valid driver's license and a reliable car available for use during work hours.</li></ul>
