Major Responsibilities<ul><li>
Assist in developing and implementing individualized treatment plans for residents.</li><li>
Supervise and support CNAs, including participation in their evaluations.</li><li>
Oversee the Falls Program and Pressure Sore Program to ensure resident safety and well-being.</li><li>
Provide essential nursing services and direct patient care.</li><li>
Conduct and document thorough medical evaluations upon admission and throughout treatment.</li><li>
Perform additional duties assigned to support the best possible care for our residents.</li></ul>
Minimum Qualifications<ul><li>
Active Licensed Practical Nurse (LPN) License in good standing.</li><li>
Experience in Skilled Nursing or Rehabilitation preferred.</li><li>
A compassionate, detail-oriented, and dedicated professional committed to excellence in patient care.</li></ul>
