 The Clinical and Admission Specialist will communicate with referral sources, staff, and community resources in transitioning patients to begin receiving home healthcare services.

 Responsibilities:
 
 
 
 <ul>
 <li>
 Conduct intake assessments and gather necessary information from patients
 </li>
 <li>
 Coordinate and schedule appointments for patients
 </li>
 <li>
 Assist with insurance verification and authorization processes
 </li>
 <li>
 Maintain accurate and up-to-date patient records in electronic health record systems such as Alora Plus, Well Sky, Axxess, and Homecare Homebase.
 </li>
 <li>
 Collaborate with healthcare providers to ensure appropriate care plans are implemented
 </li>
 <li>
 Provide support and guidance to patients and their families throughout the intake process
 </li>
 <li>
 Coordinate referrals to other healthcare professionals or community resources as needed
 </li>
 <li>
 Processes all daily patient referral and intake operations including all intake processes and policies.
 </li>
 <li>
 Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted according to contract providers.
 </li>
 <li>
 Communicates with multiple service providers to coordinate care.
 </li>
 <li>
 Follows up with patients in regards to Start of Care (SOC) &amp; services being provided to measure satisfaction.
 </li>
 <li>
 Responsible for referral related phone calls.
 </li>
 <li>
 Calls physician to obtain orders when needed.
 </li>
 <li>
 Obtains verbal orders from physician when necessary.
 </li>
 <li>
 Follows up and reports delay stages, non-admits, and any other SOC issues.
 </li>
 <li>
 Establishes and maintains positive working relationships with current and potential referral sources.
 </li>
 <li>
 Builds and monitors community and customer perceptions of the Company as a high quality provider of services.
 </li>
 <li>
 Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by the Company.
 </li>
 <li>
 Provides communication and support to Community Educators and or Community Liaisons referral sources and clients.
 </li>
 <li>
 Understands and practices agency policies and procedures.
 </li>
 <li>
 Stays current with CMS guidelines.
 </li>
 <li>
 Performs workflow and data reporting in accordance with CMS guidelines.
 </li>
 <li>
 Ensures compliance with all state and federal referral/intake regulatory requirements.
 </li>
 <li>
 Answers overflow phone calls.
 </li>
 <li>
 Maintains incoming faxes.
 
 
 
 </li>
 </ul>
 
 Qualifications:
 
 
 
 <ul>
 <li>
 Previous experience working in a healthcare setting, preferably in an acute care or behavioral health environment
 </li>
 <li>
 Familiarity with medical terminology, including ICD-10 code
 </li>
 <li>
 Experience working with children and/or individuals with behavioral health needs is highly desirable
 </li>
 <li>
 Knowledge of physical examination procedures, medication administration, and catheterization techniques
 </li>
 <li>
 Strong organizational skills and attention to detail to ensure accurate documentation and scheduling
 </li>
 <li>
 Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare providers
 </li>
 <li>

 Pay: $37.00 - $40.00 per hour
