<div>Description:

 Position Summary: This position provides professional case management services to individuals and families participating in the Denver Metro Homeless Prevention Program, with the goal of promoting housing stability and long-term self-sufficiency. The Case Manager conducts comprehensive client intake assessments, develops and implements individualized service plans, and provides ongoing counseling, crisis intervention, and supportive services. This role requires collaboration with clients and community partners, continuous monitoring and adjustment of service plans, and the use of creativity and innovation in program development and service delivery to effectively meet client needs. 
 
This position is Full-Time, grant-dependent. 37.5 hours per week. 
This position is grant-funded through two awards currently scheduled to conclude at the end of 2026. Continued employment is contingent upon the availability of future funding.

Provides information and referrals to clients.</li><li>
Conducts client intake and eligibility assessments</li><li>
Develops, implements, monitors, and modifies service plans with clients.</li><li>
Provides crisis intervention and supportive services to assist clients in reaching housing stability goals.</li><li>
Inputs client information into databases as required.</li><li>
Requests financial approval for the services of the program based on assessment of need and availability of resources.+</li><li>
Maintains accurate case records; documents client contacts and progress to ensure accountability.</li><li>
Allows new staff and interns to shadow and to sit in with client appointments.</li><li>
Works to ensure proper completion of paperwork and adherence to grant eligibility.</li><li>
Assists with reporting as needed by providing client testimonies.</li><li>
Collaborates with community partner agencies.</li><li>
Participates in other funder-required committees/workgroups as needed.</li><li>
Participates in staff trainings as required.</li><li>
Uses creativity and innovation in service delivery.</li><li>
Maintains confidentiality of client and agency information.</li><li>
Becomes familiar with and navigates assistance through all grants and funding currently being utilized by JFS.</li><li>
Assists with special department programs.</li><li>
Other duties as assigned.</li></ul>

Requirements:

 Qualifications: 
Education/Experience:<ul><li>
Preferred bachelor's degree in social work, related field, or equivalent work experience.</li><li>
Preferred three plus years&rsquo; experience as a Social Case Worker for a public or private agency.</li><li>
 Knowledge and understanding of the Denver metro area resources is preferred.</li><li>
Bilingual in Spanish is strongly desired.</li><li>
Strong communication, organizational, and interpersonal skills.</li><li>
Ability to interact effectively as a team member and independently with Agency staff and with a diverse client base.</li><li>
Ability to multitask in a fast-paced environment.</li><li>
Must be proficient in Microsoft Office Software.</li></ul>
Work Environment: Off-Site &amp; In-Office Position: Work primarily takes place in the community with occasional requirements to be at the home office at 3201 South Tamarac Dr., Denver 80231. Work hours are generally during regular business hours, with occasional flexibility required to support agency needs. 
 
Essential physical requirements: Frequent sitting and typing. While performing the duties of this job, the employee is regularly required to use standard office equipment. The employee is regularly required to travel between sites, communicate effectively, and use standard mobile equipment. 
 
Physical effort/lifting: The employee may occasionally be required to lift up to 10 pounds.</div>
