QUALIFICATIONS:<ul><li>
Minimum of a Bachelor&rsquo;s degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or a related field, plus two years of experience working with frail older adults.</li><li>
Current unrestricted CA Driver&rsquo;s License and proof of auto insurance.</li><li>
Care management and field-based service delivery experience preferred.</li><li>
Ability to target and navigate needed resources for the aging population preferred.</li><li>
Bilingual Russian, Spanish, or Farsi is preferred</li><li>
Experience conducting in-home visits is strongly preferred</li><li>
Excellent communication skills both verbal and written and ability to utilize telehealth platforms.</li><li>
Must be computer literate in MS Office applications.</li><li>
Culturally sensitive and able to work with a multicultural client/customer population; ability to work in a non-judgmental manner and foster positive outcomes; able to work as part of a team and maintain professional boundaries.</li><li>
Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment.</li></ul>
RESPONSIBILITIES/ESSENTIAL FUNCTIONS:<ul><li>
Respond to intake callers to gather initial information and provide information and referrals.</li><li>
Provide in-person home visits.</li><li>
Review monthly service delivery invoices, verify client receipt of services, and process invoices for payment.</li><li>
Provide outreach and support to clients in collaboration with the Registered Nurse as tasks are assigned</li><li>
Assist clients in applying for basic grants and other government benefits as assigned.</li><li>
Assist with the coordination of family or other informal support for clients that are assigned.</li><li>
Arrange transportation for doctor&rsquo;s appointments or assistance with other client needs.</li><li>
Process monthly food vouchers for assigned clients.</li><li>
Provide timely documentation in keeping with funding requirements and electronic health records.</li><li>
Participate in multi-disciplinary team meetings as scheduled</li><li>
Participate in weekly individual supervisory meetings</li><li>
Other related duties as assigned.</li></ul>
