 Key Responsibilities

 &middot; Track patient data from remote monitoring devices to identify trends.
 
 
 &middot; Check in with patients (by phone or in-person at the clinic) to assess progress.
 
 
 &middot; Communicate important health updates to physicians and clinical teams.
 
 
 &middot; Educate patients about medication, lifestyle, and self-management techniques.
 
 
 &middot; Document patient interactions and clinical updates in the EHR system.
 
 
 &middot; Schedule and oversee follow-up appointments and referrals.
 
 
 &middot; Help patients navigate available healthcare services and resources.
 
 
 &middot; Escalate any potential risks or health changes to providers promptly.
 
 
 &middot; Foster ongoing patient engagement to boost long-term health outcomes.
 
 
 
 
 
 Qualifications
 
 
 &middot; Current Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) certification.
 
 
 &middot; Familiarity with Chronic Disease Management and Remote Patient Monitoring.
 
 
 &middot; Proficiency in using EHR systems for documentation and care coordination.
 
 
 &middot; Excellent communication and interpersonal skills to motivate and support patients.
 
 
 &middot; Strong organizational abilities to manage multiple patients and tasks effectively.
